Oh no is right! When a child lays their head down on their table or desk like this, it usually means one thing: ❌ COMPENSATION ❌ They’re compensating for weakness or challenges. Typically relating to poor ocular motor skills, convergence insufficiency, or weak visual skills. Sometimes it’s because they didn’t crawl as a baby, sometimes it’s genetic, sometimes there’s no underlying ‘cause.’ Rarely it’s due to fatigue, especially if you notice this position immediately after they start writing. So if your child is writing like this, what do you do? ✏️ When in doubt, rule it out. ✏️ Assess ocular motor skills and primitive reflexes. Find a developmental optometrist, someone who will assess ocular motor skills, not just visual acuity. I’m taking my kids to their evaluations soon and I am so excited to see how it goes! The downside of vision therapy? Insurance typically doesn’t cover it. The upside? If you find the right vision therapist, the issues are typically remediated for good! A smooth pursuit tracking exercise where children follow a moving object (here, a capped pen) with their eyes. This activity targets visual attention, eye coordination, and tracking skills, foundational for reading, writing, and motor planning. CLINICAL CONTEXT Visual tracking exercises are integral to occupational and vision therapy protocols. However, these activities are most effective within structured, individualized treatment plans where progress is monitored and adjusted based on each child’s specific needs and developmental profile. Esotropia, or "crossed eyes," is a type of strabismus where one or both eyes turn inward toward the nose, disrupting binocular vision and potentially causing double vision or lazy eye (amblyopia). It can be constant or intermittent, appearing in infants (infantile esotropia) or developing later, often linked to genetics, farsightedness (accommodative esotropia), or neurological issues, and is treated with glasses, vision therapy, or sometimes surgery.  Causes Genetics: A family history of eye misalignment. Farsightedness (Hyperopia): Over-focusing (accommodating) to see clearly can cause the eyes to cross (accommodative esotropia). Neurological problems: Issues with the brain or nerves controlling eye muscles. Premature birth: A risk factor, especially for infantile forms.  Types Infantile Esotropia: Develops in the first six months, often constant and large-angle. Accommodative Esotropia: Triggered by focusing, common in young children. Intermittent vs. Constant: May appear only sometimes or all the time.  Signs & Symptoms Visible inward turning of one or both eyes. Squinting, blinking, or tilting the head to see better. Accommodative Insufficiency 👀 Accommodative Insufficiency (AI) is a condition where the amplitude of accommodation is lower than expected for a person’s age, making it difficult for the eyes to focus on near objects such as books, phones, or computer screens. Normally, when we look at a near object: The ciliary muscle contracts The crystalline lens becomes thicker The refractive power increases In accommodative insufficiency, this mechanism is weak or inadequate, so the eye cannot maintain clear near vision. 2. Causes Common causes include: Excessive near work (mobile, computer use) Uncorrected refractive errors (especially hyperopia) General fatigue or illness Post-viral conditions Certain medications affecting accommodation Neurological conditions such as Oculomotor nerve palsy 3. Symptoms Patients commonly complain of: Blurred near vision Eye strain (asthenopia) Headache during or after reading Difficulty maintaining focus Reading fatigue Reduced concentration during near tasks These symptoms usually appear after prolonged near work. 4. Clinical Findings (Diagnosis) Optometrists diagnose accommodative insufficiency using several tests. Key diagnostic signs Test. Finding *Amplitude of Accommodation---Reduced for age *MEM Retinoscopy----High lag of accommodation *PRA (Positive Relative Accommodation)- -------Reduced *Accommodative Facility---------Difficulty with minus lenses. These tests help differentiate AI from other accommodative disorders. 5. Management / Treatment Treatment aims to improve accommodative ability and reduce symptoms. I saw Hrithik Roshan’s post about reducing his eye number by half with vision therapy exercises — so let’s talk science. After 40, what most people experience is presbyopia, an age-related reduction in focusing power. Vision therapy and orthoptic exercises can strengthen your accommodation (your eye’s focusing muscle) and may temporarily improve how clearly you focus. But just like any other muscle in the body — if you stop exercising it, the effect reduces. You cannot permanently reverse presbyopia. At best, you may slow it down or temporarily improve focusing ability. The number doesn’t “disappear” — it may simply change. You may still need glasses — just possibly with a different power. Vision therapy has specific medical indications and can help in certain cases, but it does not eliminate refractive errors permanently.
