External fixation (anterior frame) membantu menutup rongga panggul, mengurangi perdarahan, dan memberikan stabilisasi awal sebelum tindakan definitif, dalam beberapa pertimbangan bisa dipertahankan hingga tulang menyambung sehingga tidak perlu tindakan operasi tambahan Pada kasus ini, perempuan 18 tahun, datang pasca kecelakaan lalu lintas dengan kondisi lemah, pucat dan kadar Hb yang rendah akibat pendarahan dalam. Tindakan emergensi resusitasi dikerjakan termasuk pemasangan external fiksasi anterior frame. Dengan pertolongan Allah pasien selamat dan lanjut ke proses pemulihan. Is Discharging Patients After External Fixation Safe?

Retrospective cohort at a Level 1 trauma center
178 patients screened → 102 met inclusion → 80 had ≥3 months follow-up for outcome analysis
Fracture types: 66 tibial plateau (41B/C) and 36 pilon (43B/C)
Compared patients discharged home/SNF between stages vs kept admitted after spanning external fixation. 📊 Results
Hospital course:
• Total hospital stay – Discharged: 7.1 days | Admitted: 22.6 days
• Days between stages – Discharged: 17.5 | Admitted: 10.8
• Temporary pin-site infections – Discharged: 14.3% | Admitted: 2.7% Long-term outcomes (in the 80 patients with adequate follow-up):
• Nonunion: 9.1% vs 13.7%
• Deep infection: 18.2% vs 25.9%
• Unplanned reoperation: 36.4% vs 36.2%
→ No significant differences. ⚠️ Limitations

Young Storm came to me when an unknown trauma broke the bones in his tiny front paw (the metacarpus or palm bones). Using tiny pins, I reassembled the broken bones and then held the pins in place with simple plumber’s putty. I invented this technique in the 90s. It’s called a Secured Pin Intramedullary Dorsal Epoxy Resin (SPIDER) frame – and it really does look like a spider. Sometimes simple inexpensive things really are the best and this is a really fantastic external fixation (ESF) technique which is reliable and effective for this kind of trauma – but I gotta say, it must be put on properly – that’s the key. I’ve used it on hundreds of patients and it’s been a pleasure to treat Super-Storm.
