Have you ever tried using horizontal strokes in the posterior? I recommend trying this stroke type with your curettes and see how you like it! You will need a strong fulcrum, lateral pressure, and control. Be gentle as your toe is directed towards the gingival margin. I’m demonstrating with a sickle here because it’s what I had on hand at the time (thanks @PDTdental!), but I recommend using a curet. The horizontal stroke is great for direct surfaces (buccal and lingual) and line angles. Description • A modified form of enamel hatchet with the blade curved and the cutting edge angled (not at 90° like a hatchet). • Available in pairs (right & left cutting) for use on mesial and distal margins. • Has 2 main types: Mesial GMT & Distal GMT (depending on the cutting edge angle). ⸻ Uses of GMT 1. Refining Gingival Seat – smoothens and flattens the gingival floor in Class II cavities. 2. Trimming Gingival Margin – removes unsupported enamel rods at the gingival cavosurface. 3. Creating a Proper Cavosurface Angle – ensures butt joint margin (for amalgam) or beveling (for composite). 4. Smoothing Internal Line Angles – reduces stress concentration and helps in proper restoration adaptation. 5. Beveling (Composite prep) – on enamel margins to improve bonding. 6. Gingival recession and orthodontics, which comes first: ortho or perio? Aetiology Gingival recession is multifactorial: plaque-associated inflammation, traumatic brushing, high frena, smoking, thin periodontal phenotype, iatrogenic factors and tooth malposition outside the alveolar envelope (Marschner 2025; Niemczyk 2024; Joss-Vassalli 2010). Classification Miller I–IV (Miller 1985) is simple and widely used. Cairo RT1–RT3 (Cairo et al. 2011) uses interproximal CAL and is more predictive for root coverage outcomes (Fageeh 2024). Management (how to sequence ortho and perio?) 1. Non-negotiable Stabilise active periodontitis, pockets and inflammation before applying orthodontic forces: OH instruction, scaling and root surface debridement, risk-factor control (Pinho 2025; Zhong 2025). 2. Ortho first, then perio plastic surgery Best when recession is mainly due to malposition (e.g. flared lower incisors), the phenotype is not extremely thin and teeth are being moved back into the bony envelope. After ideal alignment, root-coverage procedures such as CAF + CTG in RT1/RT2 or Miller I/II defects are predictable (Chambrone 2010; Cairo 2011; Chambrone 2024; Fleming 2024). 3. Perio soft tissue augmentation first (or combined), then ortho Consider grafting before or during orthodontics when there is a markedly thin phenotype, existing dehiscence/RT1 recession, or when significant proclination, decompensation or arch expansion is planned (Chen 2022; Li 2024; Song 2020; Chambrone 2024; Yu 2021). In short, there is no single “better” sequence: – Treat inflammation first, always. – Low-risk, moving teeth back into bone: stabilise perio → ortho → root coverage if needed. – High-risk thin phenotype/dehiscence: stabilise perio → soft tissue augmentation (or PAOO/SFOT) → carefully controlled ortho.
