What’s the cause of ITP?
Primary - idiopathic like most other autoimmune disorders. Seen more commonly in children and usually follows a respiratory tract infection. Thrombocytopenia will persist for less than 6 months so acute presentation.
Secondary - seen in adults. Thrombocytopenia persists more than 6 months. Usually seen concomitant with other autoimmune disorders or because of infections/drugs.

Cause of VWD?
Genetic -
Gene for this factor is chromosome 12. If it’s a quantitative problem , type 1 or 3. 1 is autosomal dominant and 3 is recessive and severest form.
Qualitative problem is type 2. (2a and 2b)
Acquired -
Secondary to autoimmune disorders, Myeloproliferative disorders.

So what is hess test and why’s it positive in platelet disorders?
The Hess (Rumpel-Leede) test checks capillary fragility and platelet function Because platelets are essential for:
Maintaining capillary endothelium integrity
Stopping microvascular leaks by forming tiny plugs
If there’s a problem in their function or quantity, there’s going to be petechia on deflating the BP cuff.
Procedure of doing the test :
-Inflate a BP cuff on the arm to midpoint between systolic and diastolic pressure (e.g. ~100 mmHg) for 5 minutes.
-Deflate it and look for petechiae.
-If >10–15 petechiae appear in a 2.5 cm square → positive test.
What is RISTOCETIN AGGREGATION TEST?
-It is an investigation of choice in VWD.
- checks the adhesion, not aggregation so it’s a misnomer
-Ristocetin is an antibiotic that (in vitro) causes vWF to bind to GpIb on platelets, leading to platelet agglutination.
- if vWF is missing, the above is not going to happen.
- also seen in Bernard soulier because GpIb deficiency so this is how we’ll differentiate them :
on adding normal plasma, if aggregation occurs, it’s vWD and if it doesn’t occur, problem was in the receptor.

Disseminated intra vascular coagulation- DIC
haemolytic uremic syndrome - HUS
Microangiopatgic hemolytic anemia - MAHA

🧠 Clinical Scenario
A 24-year-old woman presents with sudden onset petechiae over her legs and gum bleeding for 3 days. She had a mild viral fever 2 weeks ago.
On examination:
• Multiple petechiae and ecchymoses
• No lymphadenopathy
• No hepatosplenomegaly
Investigations:
• Hb – 12 g/dL
• TLC – 7,800/mm³
• Platelet count – 18,000/mm³
• Peripheral smear – large platelets seen
• PT & aPTT – Normal
What is the most appropriate next step in management?
Options:
A. Start IV heparin
B. Start high-dose corticosteroids
C. Immediate platelet transfusion
D. Bone marrow biopsy before treatment

MIITP (immune thrombocytopenic purpura) is the sneaky autoimmune attack — antibodies targeting platelets, causing bruising, nosebleeds, and more. Labs? Low platelets but otherwise normal. The patient can have a fever. TTP (thrombotic thrombocytopenic purpura), on the other hand, can be chaotic — ADAMTS13 deficiency leads to microthrombi everywhere. Think “FAT RN” for the clinical features, which include Fever, Anema (from Microangiopathic hemolytic anemia), thrombocytopenia, renal issues, and neurologic issues. Neuro changes are more likely to be associated with TTP while renal issues are more likely to be associated with hemolytic uremic syndrome, which we will discuss in another video. Take away? Anemia in ITP comes from bleeding, but in TTP, it’s all about the shredded RBCs.
