_EBefore modern ventilators existed, the iron lung was one of the most important life-saving machines in the world. It was a large metal chamber used to help people breathe when their muscles could no longer do the job on their own.
The iron lung became famous during the polio outbreaks of the 1940s and 1950s. Polio could paralyze the muscles that control breathing, leaving patients unable to inhale or exhale. The machine worked by changing air pressure around the patient’s body, which forced the chest to rise and fall — essentially breathing for them.
Although it looked intimidating, the iron lung saved thousands of lives and allowed many patients to survive until their muscles recovered. Some people even lived inside these machines for years.
Today, the iron lung has been replaced by smaller, more advanced ventilators. But it remains a powerful symbol of how early medical technology helped fight one of the deadliest diseases in history.

Breath holding tests can provide insight into lung health but they are not comprehensive diagnsotic tools on their own.

Clinical Presentation
Patient undergoing lung ultrasound for evaluation of suspected pneumothorax.
Ultrasound Findings
In areas of normal lung, the pleural layers slide freely, producing comet-tail artifacts (B-lines) — this is referred to as the sliding sign.
In certain regions, pleural sliding is absent, indicating separation of the pleural layers by air in the pleural space — this is the absent sliding sign.
The junction between normal sliding and absent sliding is identified as the lung point, representing the interface between aerated lung and pneumothorax.
Impression
Lung point identified, confirming the presence of pneumothorax.
Location of the lung point can provide a rough estimate of pneumothorax size.
Note: Lung point is highly specific (~100%) for pneumothorax, but may not be identifiable in all cases, particularly large pneumothoraces where sliding is globally absent.
Definition / Explanation
Lung Sliding Sign:
On ultrasound, the visceral and parietal pleura normally slide against each other during respiration, producing comet-tail (B-line) artifacts.
Presence of sliding indicates that there is no pneumothorax in that region.
Absent Sliding Sign:
Occurs when air accumulates in the pleural space, separating the pleural layers.
On ultrasound, this region shows no sliding and absence of comet-tail artifacts.
Lung Point:
The transition point between sliding lung and absent sliding, specific for pneumothorax.
Detection of the lung point confirms pneumothorax and can estimate its size.
Sensitivity is ~65%, as it may not be visible in large pneumothoraces.
Recommendations
Clinical correlation and confirmation with chest X-ray or CT if needed.
Emergency management if pneumothorax is clinically significant.
Follow-up ultrasound to monitor pneumothorax resolution or progression.
