This is the number 1 mistake I see in patients with low back pain and sciatica: They keep wanting to stretch away the pain. But stretching causes more problems because nerves HATE stretch! So instead of trying to stretch away the pain, we take our patients through a 3 phase process to get long term relief. 1) Reset pain levels through movements and treatment that will reduce inflammation 2) Addressing the root issues that led to the injury in the first place. 3) Building strength in the key muscles around the injury to lock in long-term results. Without these 3, you will keep trying to stretch away the pain, but ultimately not get any lasting results. Follow these steps carefully: 1️⃣ Bend your hip so that it makes a 90° angle with your body. 2️⃣ Put your hands behind your knee to stabilize. 3️⃣ Gradually straighten your knee while pointing your foot and toes upward. 4️⃣ Straighten your knee until tension is felt in the back of the leg. 5️⃣ Keep your leg completely still while “pumping” your foot/ankle up and down. 6️⃣ “Pump” your ankle for 5 repetitions, then bend your knee back down. 7️⃣ Repeat 10 times, to tolerance. 💡 This movement should feel like a gentle stretch and nerve glide — not sharp pain. Over 6 months of sciatica. Multiple doctors. Multiple therapists. No lasting relief. This was his first visit with us. When we evaluated him, we didn’t just look at where it hurt - we assessed the entire nerve pathway. We found multiple restrictions limiting the sciatic nerve’s ability to glide. But the worst restriction was higher than expected. At the L5 nerve roots, where they travel through the abdomen and interface with the psoas, the fascial connections were adhered. How did we know? When palpated, the tissue didn’t bow or move normally under tension. There was tone higher in the psoas - but this segment lacked normal glide. That’s a red flag. When nerve roots can’t move, the nervous system increases protective guarding downstream. That guarding sustains leg pain. In this video, I show how we release that restriction. He felt an immediate difference. But this isn’t the entire solution. Chronic sciatica is rarely one structure. It’s a process of unlayering the worst, most accessible restrictions first - calming the nervous system step by step to create sustainable change. If your sciatica keeps coming back, something is still restricting the nerve. Burning. Pulling. Deep hip pain that shoots down the leg. That’s how most people describe sciatica - and for many, it has nothing to do with a disc. In this video, I show how the sciatic nerve becomes adhered to the piriformis, limiting its ability to glide and triggering a constant protective response. Why adhesions form Adhesions develop after: • Injury or surgery • Repetitive loading • Prolonged guarding • Inflammation or immobility When tissues lose their ability to slide independently, the nervous system perceives threat. Why pressure matters Releasing a nerve adhesion is not about “digging harder.” Research and clinical models show that approximately 6.2 lbs of force is required to restore glide - but only when applied with precise contact and direction. Too little = no change Too much = guarding increases What happens when the nerve can’t move • The hip increases protective guarding • Muscles stay hypertonic • Load shifts abnormally • Pain persists - even at rest or with stretching That’s why stretching alone often makes sciatica worse. The missing piece Releasing the sciatic nerve is one step. If you don’t also: ✔️ Identify all remaining restrictions ✔️ Restore normal movement ✔️ Rebuild tolerance to load …the nervous system will protect again - and the pain comes back.
