When I started my clinics 🇺🇸, I thought I had to stay silent and just observe. But once I started asking the right questions, everything changed: doctors remembered me, offered me mentorship, and helped me grow faster. Here’s curious questions I asked that actually sparked great conversation: 1. “What other differentials should we consider and what helped you narrow down in this case?” 2. “How would this management change if the patient had [X] condition?” 3. “What signs or red flags do you never ignore in this case?” 4. “What do you wish more med students asked during shadowing?” 5. “How do you approach building patient trust so quickly?” 6. “Would you choose this specialty again if you could go back?” 7. “What’s the biggest challenge in your day-to-day practice?” 8. “How do you stay updated on new treatments and guidelines?” 9. “What do you look for in a good med student on rotation?” 10. “How do you manage difficult patients and conversations?” 11. “What made you decide between this and another specialty?” 12. “What do you think is often overlooked when treating patients?” It’s not about just trying to impress, it’s about being genuinely curious and intentional. I decided to never even ask for an LOR at the end of it I found this list a bit too late but hopefully you have found it on time!! Look for these resources and send cold emails professionally. 3. Leverage LinkedIn strategically LinkedIn is one of the most powerful tools. Consider getting premium for a short time (especially 4–5 months before Match season) and send direct messages to doctors about rotations or guidance. 4. Maximize one opportunity If you secure one rotation in the U.S., use it wisely. Network actively—talk to your preceptor and other doctors. One connection can lead to multiple opportunities. 5. Engage with community centers When in the U.S., connect with local community centers (e.g., Muslim communities). Many professionals there are willing to guide you—approach them respectfully. 6. Network within your own community Whether it’s a Pakistani community or your home-country network, these groups are often very supportive. Build connections and ask for guidance—they can open many doors.

Very proper system very proper application. You can use that. Direct apply. And….. fill a voucher of $2000+ :) NOT inclusive of boarding housing and other khana peena kharcha. So yeah ‘simple’ itna bhi simple nai :’) Then there’s contacting every family friend, relative, cousin, even people you dislike and neighbours and some person you remotely knew as a kid once and asking for help. Something works out. And that elective most likely is free. All this is NO system no proper channel no one good trick no hACK. This is a jungle and the wild. Anyone and everyone is fair game and anyone can help. Don’t even restrict it to doctors. Cold Emailing •Search for hospitals, clinics, or private practices that take students. •Email attendings, program coordinators, or department heads. •Attach your CV, a short and polite introduction, and what kind of experience you’re looking for (observership, externship, research). •Follow up after 1–2 weeks if you don’t hear back. 2. LinkedIn Networking •Search for IMGs who have done USCE and check where they did it. •Message alumni from your med school who matched into the U.S. •Politely ask about their experience and if they can recommend any programs or doctors to reach out to. 3. If You Are the Only Earning Member Be honest. The full pathway can cost: Exam fees (Step 1, Step 2 CK) OET ECFMG certification USCE rotations Travel + visa Application fees Interview travel (if in-person) It can easily cross $15,000–$25,000+ If your family depends on you financially and there are no savings, this pathway can create massive pressure. Stress kills performance. ❌ 3. If You Cannot Handle Uncertainty Match is not guaranteed. Even after clearing exams, you may not match on first attempt.
