🔹 PRACTICE QUESTION 🔹 When taking an ABG what should you do if some venous blood gets in the syringe and mixed with the arterial sample? Explain. ‼️TIP: When explaining your answer, begin with what the purpose of sampling arterial blood is. Then describe what information venous sampling provides us with. 🔸 Answer Below ————————————————————————————— 🔺Steps for acquiring an ABG🔺 1️⃣ Perform a modified Allen’s test ➡️ to assess for collateral circulation in the event a thrombus forms ✋🏼 2️⃣ Palpate the radial artery with your non dominant hand ➡️ keep this throughout the entire process until the needle has been removed as your fingers could be aiding in keeping the artery stabilized. 3️⃣ Position the needle ➡️ bevel up facing the forward flow of arterial blood 💉 4️⃣ Poke approx 2.5cm from the styloid process/wrist at a 30-45 degree angle. Break skin ➡️ pause, make sure the needle is positioned correctly above the artery. Take a moment to breath 🌡 5️⃣ Advance needle ➡️ flash of blood 🩸 6️⃣ Once desired volume as been acquired, move out in a single vector 📐 7️⃣ Tamponade for a minimum of 5min ⏳ 8️⃣ Disregard wastes into the appropriate bin 🗑 🔸 Answer 🔸 ❔The purpose of obtaining an ABG is to determine the values of PO2, PCO2 & pH. This information provides specific information on oxygen delivery. ❔Venous sampling reflects the function of local metabolic rate and perfusion. The values on PvO2 are dependent on where the sample is being taken therefore is not a good reflection of overall oxygen delivery to the cells or lung function. ❕If upon taking an arterial blood gas some of the patients venous blood (darker in colour) is mixed in with your sample, you should NOT accept this sample. Another ABG needs to drawn. Liver Function Test (LFT) Checks liver health. 🔹 Components: ALT (SGPT) ✅ 7–56 U/L AST (SGOT) ✅ 10–40 U/L ALP ✅ 44–147 U/L Bilirubin ✅ 0.1–1.2 mg/dL 🔺 High = jaundice 5. Kidney Function Test (KFT / RFT) Checks kidney function. 🔹 Components: Creatinine ✅ 0.6–1.3 mg/dL Blood Urea (BUN) ✅ 7–20 mg/dL eGFR ✅ >60 (normal kidney function) 6. Electrolyte Test Checks body balance. 🔹 Components: Sodium (Na) → 135–145 mmol/L Potassium (K) → 3.5–5.0 mmol/L Chloride (Cl) → 96–106 mmol/L 7. Thyroid Function Test (TFT) Checks thyroid gland. 🔹 Components: TSH → 0.4–4.0 mIU/L T3 & T4 → Hormone levels 🔺 High TSH = Hypothyroidism 🔻 Low TSH = Hyperthyroidism 8.
