AUHere’s how to fix it:
1. Ask for your PA4 assessment report.
Call the DWP and say: “Please send me a copy of my PA4 report.”
You need to see exactly which evidence they ignored or dismissed before you can challenge it.

2. Match your evidence to the activities.
Go through each daily living and mobility activity and link your documents to them.
Example:
“GP letter dated March 2024 confirms fatigue and memory loss relevant to preparing food, washing, and managing therapy.”

3. Clarify what the evidence actually shows.
Assessors often overlook reports because they don’t spell out function.
You can explain it in your MR:
“The occupational therapy report confirms I cannot complete personal care tasks safely without supervision.”

4. Include new or supporting evidence.
Ask your GP or therapist for a short note focused on daily function not diagnosis.
One sentence showing supervision, prompting, or risk is more powerful than pages of medical terms.

5. Stay factual in your MR letter.
Say:
“The assessor’s report states there was no evidence. I have now attached clear supporting evidence for each activity. Please review under Regulation 4(2A) of the PIP Regulations 2013.”

You’re not just re-sending paperwork you’re linking evidence directly to the tests the DWP must apply.
