Does your shoulder feel achy, weak, or irritated during pressing, lifting, or even sleeping on that side? It is often labelled “rotator cuff tendinitis.” But in most early to mid stage cases, we are dealing with rotator cuff tendinopathy which means the tendon has reduced load tolerance, not just inflammation. This usually involves: - Poor humeral head control under load - Inadequate scapular upward rotation or posterior tilt - A sudden spike in training volume or overhead activity - Weakness or delayed activation of the external rotators When the rotator cuff cannot properly centre the humeral head, the tendon becomes irritated with repeated stress. Here’s what today’s progression is targeting: - Pendulum swings to reduce irritability and introduce gentle joint motion - Cross-body adduction stretch to address posterior shoulder tightness when tolerated - Serratus roll-ups to restore upward rotation and scapular control - Seated DB external rotations to progressively load infraspinatus and teres minor - Light DB Cuban press to integrate controlled external rotation into overhead mechanics Rehab is not about avoiding movement. It is about restoring tendon capacity, improving scapular mechanics, and gradually reintroducing load.
