This is a common take, but it’s incomplete. The physician pay gap (2 million dollar lifetime difference) has been shown to persist even after adjusting for specialty, hours, and experience (JAMA Internal Medicine). So no, it’s not just about whether women negotiate. But that doesn’t mean negotiation and self-advocacy don’t matter, especially early in your career. Both things can be true: There are systemic factors And there are skills & resources we need to know, advocate for! how do we feel about negotiating?

Why are nearly 20% of women physicians stepping back within 6 years… from a career they spent 13+ years chasing? Let’s talk about what’s really happening 👇 It’s not a motivation problem. It’s not that women “can’t handle it.” It’s a system + reality mismatch. Top 3 reasons: 1️⃣ Lack of support from the medical system Limited maternity leave. Rigid schedules. Little flexibility in the years you need it most. Medicine wasn’t built with working mothers in mind. 2️⃣ The gender pay gap is still real Women physicians earn ~20–25% less than male physicians, even after adjusting for specialty, hours, and productivity. Same training. Same responsibility. Different compensation. 3️⃣ The invisible mental load Outside the hospital, women physicians spend ~8.5 more hours per week on childcare and household responsibilities than their male counterparts. So it’s not just a full clinical load… it’s a double shift. ⸻ ✨ And despite these staggering stats, I’m here to tell you: you can make medicine work for you. Maybe it’s niching your clinic to what you truly enjoy. Maybe it’s shedding the mental weight of imposter syndrome, perfectionism, and people-pleasing—so you actually enjoy your patient interactions again. Maybe it’s negotiating your worth so you can work fewer hours without a pay cut (yes, it happens more than you think). You don’t need to burn it all down. But you do need a new way to approach it. Follow me @shani_esparazmd and if you’re ready for real, doable change: I’m sharing 5 simple, realistic steps to better work-life balance for women physicians in my upcoming FREE masterclass. 👇 Comment CLASS and I’ll send you the details. ⸻ 📚 Sources: Pin JA, Pin PG.
