Ask 👩🏽‍⚕️ Ayesha: are kids contagious before they develop symptoms of gastroenteritis? Hi hello this is me asking my own question 🙋🏽‍♀️ AFTER Bodhi vommed a few nights ago, we bleached everything and were being really cautious abt contact, but what about transmission BEFORE sx (like can happen with some respiratory 🦠)?? ANSWER: Some viruses that cause gastroenteritis (like norovirus) can start shedding maybe hours before symptoms, but the window appears to be short. The bigger issue: up to ~30% of norovirus infections are asymptomatic, meaning some people will never show symptoms but still be able to spread it (albeit less effectively). Translation: if someone says “my kid is sick 🤢 but I feel fine,” that’s great … but maybe don’t share food. #askayesha #infectiousdiseases #gastroenteritis

Please stop deworming yourself. Those little pieces people are finding after a $100 parasite cleanse? It’s mucus. Fiber. Sometimes their own intestinal lining. It is not a worm. As a gastroenterologist, I can tell you with full confidence that parasites are a condition we can accurately test for in a clinic. While it is statistically exceedingly unlikely to be the issue at play, you deserve real help for your symptoms. Talk to a GI doc. And if you want to start to understand what’s actually going on in your gut, comment BOOK 🐛

Sometimes vomiting and diarrhoea isn’t as simple as an upset tummy 🐾🐱🐶 They can be signs of something mild or something serious and they often look the same at home. This is why diagnostics matter. Gastroenteritis, commonly known as the "stomach flu," is highly contagious, but you can stop its spread with a few strict hygiene and isolation measures. Healthdirect Healthdirect +1 1. Prioritize Handwashing over Sanitizer Use Soap and Water: Wash your hands thoroughly for at least 20 seconds. Why It Matters: Alcohol-based hand sanitizers are not effective against norovirus, the most common cause of gastroenteritis. When to Wash: Especially after using the toilet, changing diapers, and before handling any food or medicine. NSW Health NSW Health +6 2. Follow the "48-Hour Rule" Isolate: Stay home from work, school, or daycare until at least 48 hours after your last episode of vomiting or diarrhea. No Food Prep: Do not prepare or serve food for others during this time, as the virus can still be shed even after symptoms stop. NSW Health NSW Health +5 3. Disinfect with Bleach Bleach is Best: Many standard household cleaners don't kill norovirus. Use a bleach-based cleaner or a solution of 5–25 tablespoons of household bleach per gallon of water. Target High-Touch Areas: Regularly wipe down toilet seats, flush handles, taps, door handles, light switches, and countertops. Let it Sit: Leave the disinfectant on the surface for at least 5 to 10 minutes before wiping it away. nhs.uk nhs.uk +6 4. Handle Laundry and Waste Carefully Hot Wash: Wash contaminated clothing, towels, or bedding separately using the hottest water setting and the longest cycle available. Wear Protection: Use disposable gloves (and a mask if possible) when cleaning up vomit or stool or handling soiled laundry to avoid inhaling airborne particles. No Shaking: Avoid shaking dirty laundry, which can spread virus particles into the air. NSW Health NSW Health +5 5. Practice Personal Boundaries Don’t Share: Avoid sharing towels, flannels, cutlery, utensils, or drinking glasses while anyone in the house is ill. Avoid Public Pools: Do not go swimming for at least 2 weeks after symptoms stop, especially if the infection was caused by certain parasites. Healthdirect Healthdirect +4

Diarrhea across all ages is primarily caused by infections (viruses, bacteria, or parasites) known as gastroenteritis, along with food poisoning, medication side effects, and food intolerances. While norovirus is common in adults, rotavirus is a leading cause in children. Chronic issues often stem from conditions like IBS or IBD. . Common Causes Across All Ages Viral Infections (Gastroenteritis): Norovirus (common in adults/children), Rotavirus (most common in children), Adenovirus, and Astrovirus. Bacterial Infections/Food Poisoning: E. coli, Salmonella, Shigella, and Campylobacter. Parasites: Giardia lamblia and Cryptosporidium, often spread through contaminated water. 💊 Medications: Antibiotics, antacids containing Diarrhea across all ages is primarily caused by infections (viruses, bacteria, or parasites) known as gastroenteritis. #diarrhea #childdiet #peopleofallages #rehab

The main difference between viral and bacterial gastroenteritis (often called "stomach flu" vs. "food poisoning") lies in the cause, severity of symptoms, and duration. While both cause diarrhea and vomiting, bacterial infections are generally more severe and can lead to bloody stools. Comparison Table Feature Viral Gastroenteritis (Stomach Flu) Bacterial Gastroenteritis (Food Poisoning) Most Common Causes Norovirus, Rotavirus Salmonella, E. coli, Campylobacter Primary Symptoms Watery diarrhea, nausea, vomiting, mild cramps, low-grade fever High fever, severe abdominal pain, bloody or mucus-filled diarrhea Onset Sudden, typically 12–48 hours after exposure Can be very sudden (within hours) or take a few days Duration Usually 1 to 3 days Often 3 to 7 days or longer Transmission Person-to-person contact, contaminated surfaces Contaminated food (undercooked meat/eggs) or water Treatment Fluids and rest; antibiotics do not work Fluids; antibiotics may be prescribed for specific strains Key Distinctions Contagiousness: Viral gastroenteritis is extremely contagious and spreads rapidly in crowded environments like schools or cruise ships. Bacterial cases are more often linked to specific contaminated food sources. Stool Characteristics: Watery diarrhea is the hallmark of a viral infection, whereas blood or pus in the stool is a strong indicator of a bacterial infection. Complications: Bacterial infections are more likely to result in long-term issues like post-infectious Irritable Bowel Syndrome (IBS) or, in severe cases like E. coli, kidney failure. MUSC Health MUSC Health +3 Management and When to See a Doctor For both types, the primary goal is preventing dehydration by sipping clear fluids or using oral rehydration solutions. You should seek medical attention if you experience: Healthdirect Healthdirect +1 Symptoms lasting more than a week. Blood in your stool. A high fever (over 102°F–104°F). Signs of severe dehydration (no urine, dry mouth, extreme dizziness).

When children have diarrhea, what you give them to drink matters more than most parents realize. Water alone isn’t enough — it replaces fluids but dilutes sodium, which the body also needs. Juice and soda are worse. The high sugar content pulls more water into the gut and can actually increase diarrhea. Oral rehydration solution is the right choice. It has the correct balance of glucose and sodium to help the body absorb fluids properly. Seek medical care if your child has had no urine for 8 hours, is very lethargic, or has a noticeably dry mouth. 👨‍⚕️ ORS beats water and juice every time.

Kids throw up all the time. The real question parents ask is: when should we worry? Most vomiting illnesses in kids are stomach viruses like Norovirus. What ER doctors watch closely is whether kids are still drinking and peeing. Signs to watch • not peeing much • dry mouth • no tears when crying • very low energy Rule Vomiting or diarrhea usually isn’t the emergency. Kids not drinking or peeing much is. Save this so you remember what ER doctors watch.
