Common Causes • Tongue Placement: The tongue might stick out between or in front of the teeth instead of resting behind them. • Oral Muscle Weakness: Muscles in the tongue or mouth might not be strong enough. • Learned Habit: A child might simply learn to make the sound differently. • Physical Factors: In some cases, a tongue-tie (frenulum) or dental issues can contribute. • Habitual Pacifier/Thumb Use: Prolonged use may affect muscle development. Therapy Techniques Tongue Placement: SLPs teach precise tongue positioning, often behind the front teeth, for /s/ and /z/ sounds. Mirror Work: Watching mouth movements in a mirror helps individuals see and adjust their tongue and lip positions. Tactile Cues: Using physical guidance or touch to feel where the tongue should go. Airflow Control: For lateral lisps, exercises direct air forward, sometimes using straws or by prolonging /t/ sounds (like “t-t-t-t-t-s”). Quick overview of lisps! 👀 There are four types of lisps, but for this video, I covered the two most common types we see as SLPs: 🌟 Interdental lisps: tongue protrudes between the front teeth, causing “s” and “z” to sound like “th.” 🌟 Lateralized lisps: Air escapes over the sides of the tongue, giving a slushy sound. This type of lisp is NOT developmental, and should be treated by a speech therapist as soon as possible. The longer the child waits for correcting this, the stronger the old “habit” can be. 📈Milestones to consider: ☑️Interdental lisps (developmental in nature) can be typical until 4-5 years old. ☑️Lateralized lisps are not developmental, and should be evaluated by a speech therapist. 🧐What is a lisp? A lisp is a speech sound disorder affecting the ability to pronounce certain sounds, most commonly the “s” and “z” sounds. 👍🏻How to help at home? 🌟Observe: have the child watch you demonstrate the correct tongue placement and explain the concept of “tongue behind your teeth” or “keep your snake in the cage” 🌟Listen: have the child listen to you say it in isolation and then within a word 🌟Control: use a mirror and practice the sound together to gain control over their tongue 🌟Practice: say the sound in isolation, words, phrases, sentences and eventually, conversation! 🚨Remember: Practice makes perfect! Did you know that a lisp sometimes requires surgical intervention? A lisp is a speech impediment and a disability—not something someone can always just “learn away.” While speech therapy can absolutely help (and it helped me a lot over the years), sometimes the inner anatomy of the mouth physically prevents proper sound production, especially the S sound. In my case, that meant I ultimately needed a frenectomy to correct a tongue restriction that therapy alone couldn’t fully resolve. I experienced a lot of bullying as a kid for having a lisp, and diction has occasionally been a hurdle for me as an actor. But choosing surgery was about clarity, access, and communication—not vanity. Now, post-procedure, I already feel more freedom and precision in my speech, and I’m hopeful about how this will support my work moving forward. So here’s your reminder: mocking someone for a lisp is ableist. You’re mocking a disability. And in 2026? Absolutely not.
