PHArthroscopic Bankart repair is a minimally invasive surgery used to address shoulder instability caused by a tear in the anterior-inferior labrum (Bankart lesion). This procedure is often recommended for individuals experiencing recurrent shoulder dislocations or instability, especially in young athletes and active individuals.
Indications for Arthroscopic Bankart Repair
▪️Recurrent shoulder dislocations following an anterior dislocation.
▪️Shoulder instability (a feeling of looseness or the joint slipping out of place).
▪️Young, active individuals who engage in overhead or contact sports.
▪️Failure of non-surgical management (e.g., physiotherapy or bracing).
▪️No significant bony damage to the glenoid; otherwise, open repair or a bony procedure like the Latarjet may be preferred.
Surgical Procedure
1. Anesthesia: General anesthesia or a regional nerve block is administered to prevent pain.
2. Arthroscopic Access: Small incisions (portals) are made around the shoulder, allowing the insertion of an arthroscope (a small camera) and surgical instruments.
3. Inspection: The surgeon examines the shoulder joint to confirm the labral tear and assess for any additional damage (e.g., rotator cuff injuries or Hill-Sachs lesions).
4. Labrum Repair:
▪️Small anchors are inserted into the glenoid rim.
▪️These anchors contain sutures, which are passed through the torn labrum to reattach it to the bone.
▪️Several anchors may be used depending on the size of the tear.
5. Joint Capsule Tightening: If necessary, the joint capsule and ligaments are tightened to further stabilise the shoulder.
6. Closure: The small incisions are closed with sutures or adhesive strips, and the shoulder is placed in a sling for protection.
