It originates in the brainstem and wanders throughout the body, regulating heart rate, digestion, and respiratory rate. When the body is in a state of high stress or tachycardia (a racing heart), certain physical maneuvers can stimulate the vagus nerve to release acetylcholine. This neurotransmitter acts as a natural brake, signalng the heart to slow down and the nervous system to calm. These techniques are often referred to as vagal maneuvers. One of the most effective methods is the Valsalva Maneuver, which involves exhaling forcefully against a closed airway. A common clinical way to perform this is by “blowing into a syringe” or a thin tube for about 15 seconds. This creates intense internal pressure in the chest (intrathoracic pressure), which stimulates baroreceptors in the heart and carotid arteries. These receptors send a signal to the vagus nerve to lower blood pressure and heart rate immediately. Similarly, breath-holding or the “Diving Reflex”, which is triggered by splashing ice-cold water on the face while holding your breath, causes an immediate vagal surge. The body perceives the “cold-water immersion” as a signal to conserve oxygen, resulting in a rapid drop in heart rate. #heart #heartrate #pacemaker

It’s often performed for conditions like atrial fibrillation (AFib), atrial flutter, or certain types of tachycardia. Types of Cardioversion 1. Electrical Cardioversion: A controlled electric shock is delivered to the heart using paddles or patches on the chest. Performed under sedation to ensure comfort. 2. Pharmacological (Chemical) Cardioversion: Medications are used to stabilize and reset the heart's rhythm. Common drugs include amiodarone, flecainide, or propafenone. When Is Cardioversion Needed? Persistent atrial fibrillation or flutter causing symptoms like palpitations, fatigue, or shortness of breath. Fast heart rates (tachycardias) that do not respond to other treatments. To prevent complications like stroke or heart failure in patients with chronic arrhythmias. Before the Procedure Blood Thinners: Patients may need anticoagulants (e.g., warfarin, DOACs) for weeks before cardioversion to reduce stroke risk. Whether it’s an external pacemaker for patients fresh out of cardiac surgery or an internally implanted device, all pacemakers have a universal code to understand their settings: The first letter tells you which chamber is paced The second letter tells you which chamber is sensed The third letter tells you what the pacemaker’s response to sensing will be Internal pacemakers have a fourth and fifth letter that describe rate response and multisite pacing Pacing means the device generates an electrical impulse to trigger a heart beat, this can happen in the atria (A), ventricle (V), or there can be DUAL pacing of both chambers (A+V) Sensing is the amount of electrical activity required for the pacemaker to “see” the patient’s native heart rhythm so the device can respond appropriately. Think of it like a fence. When we raise the sensitivity, we require a higher threshold of electrical activity in order for the pacemaker to “see” it. When we decrease the sensitivity, we lower the threshold of activity required to make it easier for the pacemaker to “see.” Response to Sensing is what the device will do with what it sees! -The pacemaker can have NO response to sensing (O), meaning it will fire at a consistent rate no matter what electrical activity it sees. We use this setting in some surgeries to prevent the pacemaker from inappropriately reading electrocautery artifact as a heartbeat. -It can inhibit (I) the pacemaker from firing after sensing a native heart beat to prevent inappropriate competition. This is commonly seen in external pacemakers after surgery. Devices are often set to AAI or VVI when we want to pace, sense, and then inhibit either from the atria or ventricle.
