So… your pain might be centralized.” Cue the client’s face: “So you’re saying it’s all in my head?” As physiotherapists, we tread a fine line between explaining the science of central sensitization—how the nervous system can stay ‘stuck’ in pain mode—even after tissues have healed, and battling the stigma that centralized pain means imaginary pain. Let’s set the record straight: Centralized pain is real. It’s complex. And no, it’s not “just in your head.” It’s your nervous system doing its job… a little too well. And yes, it’s frustrating—for you and your therapist. We’re not dismissing your pain. We’re explaining a pathway to actually help you manage it. Nervous system education, movement, pacing, stress reduction—it’s not fluff, it’s neuroscience. #Physiotherapy #CentralPain #ChronicPain #PainScience #Neuroplasticity #PTLife #ItIsRealNotImagined

✨ 6 Practices That Helped Me Eliminate Chronic Pain (Yes — Eliminate) For years, I chased treatments, diagnoses, and explanations, but nothing shifted until I stopped seeing my body as broken and started retraining my brain. Here’s what truly healed me: 1️⃣ Graded Exposure & Movement Without Fear I started moving again — slowly & intentionally — not to fix my body, but to show my brain that I was safe. Step by step, I rebuilt trust with my body through physical activity, not as rehab, but as reassurance. 2️⃣ Understanding the Root Cause: My Brain, Not My Body I realized my pain was real — but it wasn’t coming from structural damage. It was a protective response from a nervous system on high alert. Learning this changed everything. 3️⃣ Nervous System Regulation Through Joyful, Safe Movement Instead of pushing through or avoiding, I chose gentle movement (like walking, dancing, stretching) to downregulate my system. Not to “fix” anything — but to soothe my overprotective brain. 4️⃣ Journaling & Releasing Repressed Emotions Pain held the messages I hadn’t been ready to hear. Journaling helped me access and release emotions that were driving the fear/pain loop. When I felt safe to feel, the pain began to quiet. 5️⃣ Somatic Tracking & Safety-Focused Awareness Rather than fearing sensations, I learned to observe them with curiosity. Somatic tracking taught me to notice pain without panic, to create space between sensation and fear — which changed my brain’s response. 6️⃣ Connection, Joy, and Awe Chronic pain thrives in isolation and fear. Healing required letting love, laughter, and wonder back in. I leaned into connection, play, creativity — because joy is medicine. #painscience #selfcompassion #mindfulness #mindbody #mindbodymedicine #neuroplastic #chronicpain #traumahealing #tensionmyositissyndrome #painscience #nervoussystemhealing #breathwork #neuroscience #somatic #anxiety #painreprocessing #nervoussystem #chronicillness #mindbodyhealing #holistic #selfhealing #tms #drsarno #fibromyalgia #howtohealchronicpain #mindbodysyndrome #neuroplasticpain #healingjourney #neuroplasticity #chronicpainrecovery

Do you see clients or are you someone who suffers from recurrent UTI pain? 💬 Have you been told repeatedly that there is no active infection in the area but you continue to have symptoms as if there are? Researchers may have found a reason why. In this specific study, they found increased neuropeptide substance P in an area of the bladder that indicated an increase in sensory nerves. Also, in a mouse model of recurrent UTIs bladders showed sensory nerve sprouting linked to elevated nerve growth factor from mast cells. 🔬🦠 The good news is, this can be treated similarly to other chronic pain conditions where your nervous system has learned to associate certain sensations with the threat of infection. ✨ here are some ideas for breaking the cycle: 1️⃣ retrain the brain: by tuning into your body in a calm state, you can retrain your nervous system to re-interpret signals coming from your body. 2️⃣ regulate your system: techniques like breath work and biofeedback can lower sensitivity thereby decreasing your response to signals coming from your body. 3️⃣ parasympathetic nervous system stimulation: lying on your back with your hands under your pelvis (I have a video on this) you can stimulate S2-4 which can increase calm signals from your pelvis specific nervous system. 🩺 Remember, pain is complex, but healing is possible. 🥶FROZEN SHOULDER🥶 Most people (and sadly most physios) still think frozen shoulder is just a “joint problem.” It’s not. ❌ New research just out highlights how it’s a multi-system condition involving hormonal, metabolic, vascular, and inflammatory dysfunction — the shoulder just happens to be the unlucky bit that cops the pain and stiffness. 😬 That’s why all the isolated treatments — manual therapy, injections, or exercise programs alone — rarely make a dent. You can’t stretch or massage your way out of a systemic health issue. We need to start thinking bigger than the shoulder: 🧠 Screen for thyroid and metabolic issues 💓 Address cardiovascular health 🍎 Improve lifestyle, diet, and stress 💪 Keep the shoulder moving and loading as tolerated The takeaway? Stop blaming the joint. Start treating the human. #FrozenShoulder #AdhesiveCapsulitis #TheSportsPhysio #EvidenceBasedPractice #Physiotherapy #ShoulderPain #PainScience #Rehab #ManualTherapy #ExerciseTherapy #MultidisciplinaryCare #PhysioEducation

Numbness or tingling is not always from a pinched nerve. Herniated discs and stenosis get a lot of attention, but nerves can misbehave for other reasons. They can become hypersensitive from inflammation, diabetes, or old injuries. Fascia can restrict their glide. Inflamed tissues can chemically irritate them. Even the brain and spinal cord can amplify signals through central sensitization. Poor circulation can cause it too. The only way to fix numbness and tingling for good is to find the exact cause, not just assume it is compression. #FRI #DrKohen #NerveHealth #NeurologyFacts #PainScience

Do you know the true history of MRI? The invention of the magnetic resonance imaging (MRI) machine has been a major advance in medicine. When it emerged in the 1990s, when I was starting my career, it showed detailed images of the inside of the body thanks to a miracle of scientific innovation. Now, doctors could easily diagnose tumors, abscesses, bleeds, cysts, and other clear-cut medical problems by non-invasive means. Surgeons began to rely heavily on MRI imaging (as well as computerized tomography (CT) scans). When it came to the diagnosis and treatment of neck and back pain, doctors, PTs, and other clinicians, were eager to apply MRI technology to musculoskeletal pain. At first it seemed to work well. People with back, shoulder, hip, knee, and foot pain were found to have many abnormalities on their MRI scans. Relatively few people were found to have tumors or infections in or around the spine. However there were tons of people who had disc space narrowing, degenerative disc disease (DDD), bulging or herniated discs, spinal stenosis, facet arthropathy, and a host of other changes. I noticed that radiologists dutifully dictated these findings into reports with increasing detail. We could now literally see what was causing the pain, or so we thought. Now we know this assumption couldn’t be further from the truth. Fuck the feelings you don’t like, if you actually change how you operate, AND operate better the bits you don’t like or that feel off have to be affected. If you already understand that the body operates and responds perfectly, every time, to the things that happen it, the you would also know: ⁃ Signals get sent to the brain, part of the body-to-brain feedback loop ⁃ that say “discomfort”, feeling of tightness, pain and every other sense-able sensation. The feeling is there because you are/are being in a way that it needs to be there. Cut your hand the body heals without conscious effort. That pain is there for that reason Shoulders tight? That may be because the whole system doesn’t feel safe going any further into that particular range of motion - it’s a built-in safety mechanism. Reversible though. Spend time at deep end ranges, the body can start to feel and BE safer there. You can’t jump to a height you body and mind feel like they could safely land from. Your senses and sensors are there for a reason. They read feelings and deliver that data to the brain, where YOU get to be in more change of the message that goes back down. You put yourself in the feedback loop between area/sensation and the brain response. YOU GET TO LOOK AT THE DATA GOING TO THE BRAIN AND SEE IF OR DECIDE, if it’s as bad or good as it feels If you ARE in it. The “lees than good” position, and you have repeatedly experienced yourself doing something harder than what you’re currently in or asking yourself to do, effectively, then you are more likely to NOT have stoppages or resistance. Less will be trying to “protect” because it doesn’t need to - because you showed it it doesn’t need to. The signals you sense are there for a reason. Build or change yourself in enough of a way that those signals no longer need to be there. #humanperformance #humanmovement #movebetter #wholebodytraining #painscience
