RNThere is a lot that happens with diabetic ketoacidosis, but here is an easy & quick break down of what is happening in the body.
𝗣𝗿𝗲𝗰𝗶𝗽𝗶𝘁𝗮𝘁𝗶𝗻𝗴 𝗙𝗮𝗰𝘁𝗼𝗿𝘀
▪️Insufficient Insulin: Missed insulin doses/non compliance/inadequate insulin therapy.
▪️Infection: Infection is a very common precipitating factor that can contribute to DKA.
▪️Undiagnosed Diabetes: DKA may be the first sign of undiagnosed diabetes.
▪️Trauma or Surgery: Physical stress from trauma or surgery can increase the body’s demand for insulin.
𝗛𝗲𝗿𝗲 𝗶𝘀 𝗮 𝗳𝘂𝗿𝘁𝗵𝗲𝗿 𝗯𝗿𝗲𝗮𝗸𝗱𝗼𝘄𝗻 𝗼𝗳 𝘄𝗵𝗮𝘁’𝘀 𝗵𝗮𝗽𝗽𝗲𝗻𝗶𝗻𝗴 𝗶𝗻 𝘁𝗵𝗲 𝗯𝗼𝗱𝘆:
▪️Insulin Deficiency: DKA often begins with a shortage of insulin. Insulin is crucial for glucose uptake by cells.
▪️Increased Glucose Production: Without sufficient insulin, glucose uptake by cells is impaired. Consequently, the liver compensates by increasing glucose production through a process called gluconeogenesis. This elevates blood glucose levels.
▪️Breakdown of Fats: In the absence of insulin, cells cannot utilize glucose for energy. Instead, they shift to using fats for fuel. This leads to increased breakdown of fatty acids, resulting in the production of ketone bodies (acetoacetate, beta-hydroxybutyrate, and acetone).
▪️Ketone Body Accumulation: Ketone bodies, acidic in nature, accumulate in the blood. As their concentration rises, the blood becomes more acidic, a condition known as acidosis. The low pH impairs normal cellular function.
▪️Dehydration and Electrolyte Imbalance: High blood sugar levels draw water from cells, causing dehydration. Additionally, increased urination leads to the loss of electrolytes such as Na, K, & Cl.
▪️Osmotic Diuresis: Hugh glucose levels exceed the kidney’s capacity to reabsorb glucose. Glucose spills into the urine, dragging water along with it, resulting in excessive urination and dehydration.

KRDKA vs. HHNS: Quick Comparison
DKA is known for:
-Abrupt/rapid onset
-More commonly seen in T1DM
-Serum glucose levels between 300-500 (not always!)
-Acidic or “fruity” smelling breath (these are the ketones!)
-Kussmaul respirations (NCLEX alert 🙂)
HHNS is known for:
-More gradual onset
-More commonly seen in T2DM
-Serum glucose levels 600mg/dL or above

𝗥𝗲𝗵𝘆𝗱𝗿𝗮𝘁𝗶𝗼𝗻: DKA often leads to excessive urination and dehydration. Normal saline bolus’s followed by continuous fluids help rehydrate the patient. We do this to correct the dehydration and dilute the excess sugar in the blood, bringing down blood sugar levels. Dehydration can lead to low blood pressure and decreased organ perfusion as well.
𝗜𝗻𝘀𝘂𝗹𝗶𝗻: Insulin helps lower blood sugar levels by allowing cells to use sugar for energy. Regular insulin is given via an IV infusion to lower blood sugar levels gradually. Insulin also inhibits the production of ketones. The goal is to bring down the high blood sugar and reverse the process of ketone production. Without insulin, ketones continue to accumulate, which can lead to ketoacidosis and acidify the blood, potentially causing organ damage.
𝗘𝗹𝗲𝗰𝘁𝗿𝗼𝗹𝘆𝘁𝗲 𝗥𝗲𝗽𝗹𝗮𝗰𝗲𝗺𝗲𝗻𝘁: DKA can lead to imbalances in electrolytes like potassium, sodium, and bicarbonate. Keeping a close eye on electrolytes is important because electrolyte imbalances can cause things such as cardiac arrhythmias, muscle weakness, & other complications.
𝗧𝗿𝗲𝗮𝘁 𝘁𝗵𝗲 𝗨𝗻𝗱𝗲𝗿𝗹𝘆𝗶𝗻𝗴 𝗖𝗮𝘂𝘀𝗲: This often means treating an infection or illness that triggered DKA. Sometimes, DKA occurs due to missed insulin doses or undiagnosed diabetes.
𝗖𝗼𝗿𝗿𝗲𝗰𝘁𝗶𝗼𝗻 𝗼𝗳 𝗔𝗰𝗶𝗱𝗼𝘀𝗶𝘀: DKA leads to a build-up of acidic ketones in the blood, causing the blood to become more acidic. As insulin therapy begins to work, the body starts to shift from burning fat for energy (which produces ketones) to using glucose. This reduces the level of ketones and gradually corrects the acidosis.
𝗧𝗼 𝘄𝗿𝗮𝗽 𝗶𝘁 𝗮𝗹𝗹 𝘂𝗽!: Elevated blood sugar and the accumulation of ketones can lead to severe acidosis, dehydration, electrolyte imbalances, and multi-organ dysfunction. In extreme cases, DKA can result in coma and death.
