The pivot shift test is important clinically as it helps determine the integrity of the ACL and overall stability of the knee joint. Here’s why we love the Pivot Shift: Dynamic Test: The pivot shift is a “moving” test” taking the knee into positions where ACL injuries often occur, similar to the pivoting and cutting motions in soccer, basketball and skiing. Unlike static tests, this one really shows you what is happening in the knee joint with movement. Tibial Subluxation: It specifically checks if there’s an abnormal amount of forward movement of the tibia relative to the femur - tibial subluxation. This along with the the feeling of “giving way” during the test provide good insights into whether the ACL is partially or completely torn. Better Treatment Decisions: While MRIs are valuable for confirming ACL injuries, they don’t always tell us if the knee is stable. A good pivot shift along with other functional tests helps guide whether conservative management or surgery is likely to be needed to restore knee stability and get the athlete back to their desired level of activity. Hold the patient’s leg at the calcaneus and apply internal rotation to the tibia. Some sources also recommend incorporating an axial compression force at this point. With your opposite hand, apply a valgus stress from the posterior side of the fibula. This maneuver causes the tibia to sublux anteriorly in relation to the femur while the knee remains extended. Next, slowly flex the patient’s knee. A positive test is indicated when the tibia suddenly shifts or “jogs” backward around 30–40° of flexion, as the iliotibial band tightens and reduces the subluxation. The Pivot Shift Test: Understanding Dynamic Knee Instability The Pivot Shift Test is one of the most reliable clinical exams for detecting ACL insufficiency, particularly because it reproduces the dynamic instability patients experience during activity. But what’s really happening biomechanically? 🔬 Mechanism: • With the knee extended, the tibia is subluxed anteriorly due to the lack of ACL restraint. • As the knee is gradually flexed while applying valgus and internal rotation force, the iliotibial band (ITB) shifts from an extensor to a flexor - beautifully appreciated in this patient. • Around 30–40° of flexion, the tibia suddenly reduces back into place, creating the characteristic “pivot shift.” 🎯 Why It Matters: • A positive pivot shift indicates functional instability, which is a major factor in deciding surgical intervention. • Unlike the Lachman test (which assesses pure translation), the pivot shift replicates the rotational instability seen in cutting and pivoting sports. • The test is only valid if the MCL is intact since medial instability can mask the shift. 🦵 Clinical Tip: A subtle pivot shift can sometimes be hard to appreciate. Have you ever heard of the Lever Sign Test aka the Lelli’s test to check the integrity of the ACL? Clinicians tap in and let me know what you think of this test / if you use it below 👇 Negative test: When the ACL is intact the downward force applied over the quadriceps will cause the heel to rise. Positive test: With a damaged ACL the downward force will cause anterior translation of tibia in relation to the femoral condyle. So, the heel will not rise! I think I read somewhere that it has incredibly high sensitivity and specificity and from clinical experience it’s been right for me 100% of the time. Why use this? it’s one of the EASIEST tests you can do as a clinician & it’s way easier to get the patient to relax to get an accurate result especially when compared to a Lachman’s or Pivot Shift test. We still also used the Anterior Drawer test for this patient which was also positive just FYI I will even have patients do this on themselves when I suspect an ACL tear over telemedicine calls to rule in/out my suspicions. 🆘ACL Tear Testing🆘 Lelli Lever Test The Lelli test or lever sign is a newish ACL test that came out with very strong research that showed excellent sensitivity and specificity. That original research was actually done by Lelli himself so it was probably skewed a little bit. 💯💯 However, it still proves to be a good test within your cluster of signs and test to determine the extent of a knee injury and specifically ACL. To do the test: 1. The patient lies, supine with the knees fully straight 2. The examiner puts one fist under the proximal third of the calf 3. The examiner then takes the other hand and pushes directly down on the distal quad of the same knee 🆘A positive test is the heel not coming off the table. 💪A negative test is the heel coming off the table. Anecdotally in doing this test I have found there are some false negatives, and I am yet to find a false positive. This is supported by research that shows the test to be. 63% to 100% sensitive of finding an ACL tear but 90 to 100% specific. That is to say the test will miss some ACL tears and give you a false negative depending on the phase and type of tear but when you do get a positive, it is usually specific to having an ACL tear. However, with special testing prior to imaging after knee injury having multiple tests such as the Lachman’s test, anterior drawer test, pivot shift test, valgus stress test at 0°, and now add the Lelli test give you a greater certainty of what diagnostic imaging may find.
