INLip Assessment ⚠️ I had patients messaging me asking what this was in my story. The “bulge” that pulsating on her upper lip is a visible branch of the superior labial artery.
It’s crucial to do a thorough assessment before treatment. If filler is inadvertently injected into the artery it could compromise blood flow causing skin necrosis (death). It’s also safer to inject the lip more superficially and avoid the wet dry borders. With this patient, I avoided that area altogether and used a 25g cannula for the rest.

DRWhat is an Open Bite?
An open bite happens when the upper and lower teeth don’t meet when the mouth is closed, leaving a
gap. This can happen in the front or back teeth.
This can happen due to extended pacifier use, thumb sucking, or the way a person swallows. When a child pushes their tongue against their teeth while swallowing or speaking, it can slowly push the teeth out of place, causing an open bite. Thumb sucking does something similar. If a child sucks their thumb for too long, it can push the front teeth forward, creating a gap. This is especially true if the habit continues when the permanent teeth are coming in.
Using a pacifier excessively also causes an open bite. The sucking action and pressure from the pacifier can change the shape of the dental arches and move the teeth out of alignment. As an airway pediatric dentist, I create custom appliances and work with therapists to prevent these issues, or treat them if they’ve occurred.

DR👉 Mouth opening — at least 3 fingers.
👉 Mallampati — if you can’t see the uvula, be cautious.
👉 Thyromental distance — less than 6 cm means trouble.
👉 Neck extension — limited movement makes laryngoscopy harder.
👉 Upper Lip Bite Test — if the patient can’t bite the upper lip, mandibular movement is restricted.
Remember: no single test is perfect, but together they give you a strong warning. Difficult airways shouldn’t be surprises — assess before you attempt.”
