Lately I’ve been contemplating whether general practice, as it currently exists, is sustainable for me. In my previous post I spoke about the moral distress of being a GP, constantly torn between wanting to offer a good, thorough consultation and watching the clock tick down ⏰ I once believed GP was the perfect avenue: continuity of care, flexibility, work life balance.. But now, as a seasoned GP with a solid run of experience, I’ve changed my mind. Being a GP is damn hard. We work within a 10 minute appointment system, despite the same BMA contracts recommending 15 minute appointments for safe practice. The reality is many practices simply cannot afford this due to overwhelming patient demand. Within those 10 minutes we are expected to: • take a full history • examine • diagnose • arrange referrals and investigations • prescribe safely, often for patients on multiple medications • safety net red flags And then repeat this process again and again, every 10 minutes, knowing every patient is different. Some take longer to get into the room due to mobility issues. Some come with complexity that simply doesn’t fit into a stopwatch. That level of cognitive load, over and over again, is immense. I haven’t fully decided whether I’ll be leaving general practice altogether. But I do know this: I want to experience acute medicine again. Because being a doctor isn’t the problem. Being a GP isn’t the problem. Conforming to an absurd 10 minute appointment system is. There are many avenues GPs can explore while remaining on the GP register and I’ll be sharing my journey as I work through what’s next 🩺 If you’re a GP who’s struggling: please know you’re not weak, failing, or ungrateful. Taking time out to gain clarity on what is burning you out is powerful, because only then can you decide what truly needs to change 🤍 #nhs #gp #generalpractice #bma #doctor
