This visual illustrates the progression of acne lesions inside the pilosebaceous unit: 1. Excess sebum Sebaceous glands become overactive, often under hormonal or inflammatory influence. Increased lipid production fills the follicle, creating an environment prone to blockage. 2. Comedone formation Abnormal keratinization causes dead keratinocytes to accumulate instead of shedding normally. Sebum + compacted corneocytes form a microplug, leading to open or closed comedones. 3. Micro-cyst As the follicle remains blocked, pressure builds. The follicular wall stretches, and inflammatory signaling begins — often before anything is visible on the surface. 4.
