“A common cause of hyperpigmented patches on the neck.” This patient presented with multiple asymptomatic hyperpigmented patches with fine branny (furfuraceous) scales over the neck. On gentle scratching, the fine powdery scaling becomes more apparent — a useful bedside clue. This presentation is typical of Pityriasis (Tinea) versicolor, a very common superficial fungal infection seen frequently in clinical practice. It is caused by Malassezia species, a lipophilic yeast that normally resides on human skin but can proliferate under favorable conditions such as: • Hot and humid climate • Excess sweating • Oily skin • Occlusive clothing The organism produces substances like azelaic acid, which interfere with melanin production, leading to hypo- or hyperpigmented patches with fine scaling. Diagnosis is usually clinical, but can be confirmed with: 🔬 KOH mount: showing short hyphae and spores (“spaghetti and meatballs” appearance). Treatment Most cases respond well to topical antifungals, such as: • Ketoconazole shampoo/lotion • Selenium sulfide lotion • Topical azole creams For extensive or recurrent cases, oral antifungals (itraconazole or fluconazole) may be prescribed. ⚠️ Even after treatment, pigment changes may take weeks to months to normalize. Prevention • Reduce excess sweating • Avoid tight occlusive clothing • In recurrent cases, periodic antifungal washes may help reduce relapse. #Dermatology #versicolor #ClinicalDermatology #SkinInfection #haldwani
