FAThe new ACC/AHA dyslipidemia guidelines were released yesterday, and after going through the full document there are some major themes many clinicians have been waiting for.
Key updates include:
• LDL treatment targets are back
• Risk assessment recommended for adults age 30–70 using PREVENT-ASCVD
• One-time screening for Lipoprotein(a)
• ApoB emphasized as a marker of atherogenic particle burden
• Expanded role for coronary artery calcium scoring
The overall message is clear:
identify cardiovascular risk earlier and reduce lifetime exposure to atherogenic lipoproteins.
Also notable — the guideline does not recommend HDL targets, cholesterol ratios, or pharmacologically raising HDL.

WHAlthough I knew I wanted to be a PA from day one in undergrad, there’s one thing I truly wish I had done before starting PA school: become a medical assistant.
I had solid experience before school, but looking back, working as an MA would have given me a much deeper understanding of what a real clinic day actually looks like. THSo what’s going on with our 34 year old female with monocular vision loss?
If you said retinal migraine, that’s the leading diagnosis. But here’s what most people miss: this is a diagnosis of exclusion.
You cannot make this diagnosis in urgent care without ruling out serious vascular causes first.
Retinal or ocular migraine causes transient visual loss in one eye only with or without headache. The key feature is that symptoms are fully reversible and happen the same way every time.
Up to half of patients with retinal migraine can develop permanent vision loss if untreated.
This patient needs urgent evaluation to rule out carotid stenosis or dissection, cardiac workup to look for embolic sources, and ophthalmology referral within 24 hours.
Even in young patients, 15% will have a vascular abnormality that needs treatment.
Only after excluding diagnosis like stroke, TIA, retinal detachment, and arteritis can you diagnose retinal migraine.
Management includes migraine prophylaxis to prevent permanent vision loss, even with infrequent attacks.
So remember….
Monocular vision loss is never just migraine until you prove it. Always think vascular first.
