BHThere are many things in medicine I can accept…
but the primary–secondary care interface?
Some days it feels like it was designed by a committee of people who have never met each other.
1. Signposting to services that are already full
We’re told to “redirect patients appropriately”…
Except half the services we redirect them to aren’t accepting referrals, have 12–18 month waits, or simply send the patient straight back to us.
Make it make sense.
2. Hospital letters that say a lot… without actually doing anything
A beautifully worded clinic letter that ends with:
“Please can the GP organise all investigations, prescriptions, chasing and long-term management.”
Ah yes — thank you for this generous gift of extra work.
3. Mental health crises that fall into the void between services
Too high-risk for primary care,
not high-risk enough for secondary care.
And we’re left holding the entire safety net with zero support.
Honestly, how is this a system?
These are the parts of medicine that truly make no sense to me — and yet somehow, we’re expected to carry the weight of all of them.

MOSecond clip from last night’s live commentary and Q&A re ongoing votes to end the shutdown and what can be done about it for those who are on the front lines. Federal employees and their families deserve to be treated with dignity and respect. Their livelihoods should not be trapped in the cross hairs of political war games. They should not be forced to work for no pay in order to keep their jobs nor furloughed and standing in food bank lines while they are gainfully employed by the wealthiest country in the world. And IF they are being called upon to make those kinds of sacrifices then it has to be worth it. And all those calling upon them to do it should be ready and willing to step up and shoulder the burden collectively. Short version: put your money where your mouth is. And frankly, even that is ridiculously complicated because there are stringent rules surrounding federal workers accepting monetary benefits.
This administration does not care about the federal workforce. The director of the OMB stated plainly that he wished to cause federal employees so much trauma that they would no longer want to work. We should not help them accomplish this objective.
