It’s can be understood better with the Craig’s Test. And it’s something that we can’t change (structural) so for him, we encourage him to turn his feet in. Hips don't lie... especially when it comes to the gait cycle. 😎🚶🏻‍♀️  Trendelenburg gait is the dropping of one hip when you are walking. Keep an eye on this patient's hips, watch the distance between the outside of her right hip and the arm of the treadmill. Then do the same on the left. Do you see a difference? 🕵️ ➡️ When she is standing on her right leg, her right hip is not doing a good job of keeping her hip in alignment which is why her left hip drops down and her right hip moves out to the side.  So now that we see it in the gait cycle, let's assess it. This foam roller hip hike is a great way to see if we CAN get that hip to engage. Often, this is a pretty big struggle on the side that's having a hard time.  Even better... this also makes for a great exercise!! Gait analysis can be difficult, but it’s 100x easier with high resolution slow motion video! Most of what I am showing you is nearly impossible to witness unless you have the proper angle and speed. Thank goodness for technology. I would say I have pretty “normal” gait mechanics with nothing standing out as abnormal. Additionally I don’t have pain anywhere from my feet to my low back. (My neck and thoracic spine are a different story but I think that’s due to how much time I spend making content 😂😂) Take care, God Bless!

This video is a gait analysis of one of my patients as she returns to running after rehabbing a metatarsal stress fracture. Supination is a natural part of the gait cycle, but excessive supination—where the foot consistently lands on the outer edge—can reduce shock absorption and increase injury risk. Efficient running relies on a smooth transition between supination and pronation to properly absorb and distribute impact forces: In sprinting, the foot typically lands on the forefoot or midfoot, and a slight degree of pronation helps absorb shock while maintaining stiffness for explosive propulsion. In distance running, controlled pronation is essential for shock absorption. It spreads impact forces across the entire foot and up the kinetic chain, reducing stress on the outer foot, ankles, shins, and knees while promoting a more efficient and balanced gait. Several factors can contribute to excessive supination: ✔ Limited Foot & Ankle Mobility – Restrictions in dorsiflexion or eversion (often due to ankle stiffness or high arches) ✔ Weakness in the Lower Leg & Foot Muscles ✔ Limited Big Toe Mobility & Strength ✔ Compensatory Gait Patterns – Past injuries, bunions, or movement habits can reinforce an over-reliance on the lateral foot, often as an unconscious strategy to avoid pain or instability. To improve adaptability and reduce stress on the lateral foot, we focus on: ✔ Improving Foot Flexibility & Pronation ✔ Big Toe Extension & Plantarflexion ✔ Hip, Knee, and Foot/Ankle Strength & Neuromuscular Control The goal isn’t to eliminate supination—it’s to enhance adaptability so that she can transition efficiently between supination and pronation. This will help reduce injury risk, improve force distribution, and enhance overall running efficiency. Walking is a whole-body task. Arms swing, the trunk rotates, the foot adapts to the ground, and the nervous system coordinates the lot. But this clip is deliberately isolating the lower limb so you can actually see what’s happening there instead of everything at once. In the hip you’ll see activity from muscles such as gluteus maximus and medius helping control hip extension and pelvic stability during stance. As the leg moves forward, hip flexors like iliopsoas contribute to limb advancement. In the thigh, quadriceps control knee flexion during loading and help extend the knee as you move over the foot, while the hamstrings assist with hip extension and decelerate the leg in late swing. Down the leg, gastrocnemius and soleus contribute to push-off, and tibialis anterior controls the foot as it lowers to the ground. The point of the video isn’t to say these are the only muscles involved. It’s to visualise lower-limb contribution during gait. When you strip it back like this, you can better appreciate load transfer, shock absorption, and propulsion through the hip, knee and ankle.

Overpronation = injury? 🤔 Not necessarily. Many runners overpronate and never get hurt—including me! Research shows that foot roll alone is a poor predictor of injury. ✅ Injuries are much more often linked to: • Sudden jumps in mileage or intensity 🏃‍♂️ • muscle weakness and lack of tissue capacity 💪🏻 • Fatigue and training errors ⚡ Overpronation isn’t inherently a problem if you: • Build tissue tolerance – gradually strengthen muscles, tendons & joints to handle forces • Strengthen the chain – strong calf, hips, glutes & core keep your biomechanics solid So no. Overpronation doesn’t cause your injury in isolation. It doesn’t mean you need to buy a stability shoe and you don’t need to ‘Fix’ it. Low Cadence means OVERSTRIDING? People assume that if someone runs with a lower cadence, they MUST be over striding. Instead of just focusing on the numbers, LOOK at the person running. In this video, I use Jim Walmsley as an example. Here, he presents with a cadence of 165 steps per minute, which can be considered relatively low. But is he actually overstriding? You will see that it lands relatively close to the body, even though he has a lower cadence. Now, let me clarify something... Increasing one's cadence or step rate as they run can still be beneficial to help one learn to land closer to the body. It can also be a helpful strategy to reduce peak forces with each step you take, especially returning from injury. HOWEVER... Perhaps you should still learn to get that foot to land closer to the body no matter how high or low the cadence is. - This is for educational purposes and should not replace medical advice. If you are dealing with persistent running related injuries, I recommend seeing a healthcare professional. - #running #cadence #gaitanalysis #biomechanics

When a runner can’t control pronation, the foot will compensate because pronation is a necessity during mid stance of gait. In some cases you can have pain on the inside of the ankle or even outside of ankle depending how the body compensates Over pronation is really just eversion of the foot, not true pronation. Over time this can even cause a bunion Over pronation doesn’t mean you’re going to have foot or ankle pain but it can contribute. If you are having pain, I recommend going through the Foot and Ankle Rehab Program for running specific rehab exercises to improve your biomechanics If you’re in Richmond, Virginia, I’d love to work with you in person through a gait analysis and full body assessment with @gritphysicaltherapy
