Googling “scleroderma” is terrifying — because it sounds like one diagnosis. It’s NOT. Scleroderma is an umbrella term, not a single disease. There are localized forms like morphea or linear scleroderma, which affect the skin and underlying tissue and do not involve internal organs. There are also systemic forms, called systemic sclerosis, which include limited disease (often called CREST) and diffuse disease. These are the forms that can involve internal organs and require closer monitoring. This distinction matters — because prognosis, testing, and treatment look very different depending on which type someone has. As a rheumatologist, I don’t diagnose scleroderma based on one lab. When systemic sclerosis is suspected, I look at: • Symptoms and physical exam (skin thickening, Raynaud’s, joint findings) • Antibody testing such as anti-centromere, Scl-70, RNA polymerase III • Inflammatory markers and baseline labs (CBC, CMP, ESR/CRP) • Lung screening when appropriate (PFTs ± HRCT) • Patterns and changes over time Google often shows worst-case scenarios. Before & after my Scleroderma diagnosis 🌻 I’m going to make this a short post because it has been one of the hardest videos to make. On the positive side it is good reminder that regardless of my Scleroderma diagnosis my body has been constantly changing. We tend to get attached to our looks forgetting that we are not going to be young forever. We are all going to change as we get older 😜 My transformation is just a little bit different. Systemic Scleroderma is an autoimmune disease that attacks the connecting tissue. Most of your body is made up of connective tissue so that means it can attack 99% of your body. Not everyone has the same progression. Some have more internal issues and others more external. The thing is that change is inevitable so it’s better to accept the reality of your condition than to keep fighting it. I think the saddest part about this post is knowing that the woman before my diagnosis was so insecure about her looks. I wish I could go back and hug her and let her know she was and is still beautiful 💕🌺 & she was stronger than she thought she was. Now I’m here and I’m still trying to get used to my new looks but I’m not angry anymore. If you are living with scleroderma, these questions are very common. Let us walk through them together. 1️⃣Will I pass this on to my children? It is extremely unlikely. Even identical twins who share the same DNA only have about a 5 percent chance that both will develop scleroderma if one twin has it. That tells us genetics alone do not explain this disease. There are many other environmental and immune factors involved. 2️⃣If my antibody test is negative, does that mean I do not have scleroderma? No. You can still have scleroderma without a disease specific autoantibody. There are more antibodies identified today than in the past, and sometimes only one or two are checked. Diagnosis is based on the full clinical picture, not one lab result. 3️⃣Can stress trigger flares? Based on my clinical experience, yes. Overwhelming stress can worsen symptoms, including skin changes, lung symptoms, and gastrointestinal flares. This is why supporting the mind body connection is not optional. Systemic sclerosis, also known as scleroderma, is a chronic autoimmune disease causing fibrosis (hardening) of the skin and internal organs (lungs, heart, kidneys, GI tract) due to excess collagen, often starting with Raynaud's (finger color changes) and leading to swelling, skin thickening, and potential organ failure . It's categorized as limited (milder, below elbows/knees) or diffuse (widespread skin/organs) . While the exact cause is unknown, it involves immune system dysfunction, genetics, and environmental triggers, with prognosis varying by type, but lung/heart/kidney issues are major mortality risks . Key Characteristics & Symptoms Autoimmune: The immune system mistakenly attacks the body's own tissues. Fibrosis: Overproduction and buildup of collagen, making tissues hard. Vasculopathy: Damage to small blood vessels. Common Early Signs: Raynaud's phenomenon (fingers/toes turn white/blue), swollen hands/fingers, joint pain, heartburn. Progression: Skin tightens, spidery blood vessels (telangiectasias) appear, and internal organs can be affected.  Types of Systemic Sclerosis Limited SSc (lcSSc): Affects skin on face, hands, feet, lower legs; slower progression; can involve CREST syndrome (Calcinosis, Raynaud's, Esophageal dysmotility, Sclerodactyly, Telangiectasias). Diffuse SSc (dcSSc): Affects skin on trunk and limbs; rapid onset; higher risk of severe internal organ involvement. Organ Involvement & Complications Lungs: Pulmonary hypertension, fibrosis. Kidneys: Scleroderma renal crisis (sudden high blood pressure, kidney failure). Heart: Heart failure, arrhythmias. GI Tract: Difficulty swallowing (dysphagia), heartburn, motility issues.  Diagnosis & Management Diagnosis: Clinical signs, autoantibody tests (like ANA). Treatment: Focuses on managing symptoms and specific organ complications, using immunosuppressants, blood pressure drugs, etc., with ongoing monitoring.
