Corns, callus and bursae are often misunderstood. They are not simply problems to remove, they are protective responses from the body. A bursa forms deep within the tissue as a cushioning mechanism to protect bone and soft tissue from excessive pressure. It develops in response to stress and helps prevent damage. Callus forms at the surface due to friction and pressure. It thickens the skin to act as a natural barrier. When pressure becomes more intense, the body produces a corn. A corn is highly concentrated keratin, similar in structure to nail, forming a dense plug that protects the underlying tissue from breaking down into an ulcer. These three can exist at the same time, working together as a hierarchy of defence to prevent tissue breakdown. The key issue is not their presence, but the cause. If pressure, footwear or biomechanics are not addressed, they will continue to develop. In patients with reduced sensation, such as neuropathy, these protective mechanisms may not form effectively. This increases the risk of ulceration. The foot is constantly adapting to protect itself. Housemaid's Knee:- It is otherwise called Prepatellar bursitis as there is inflammation of Prepatellar bursa. Bursae are the small fluid filled sacs present all over the body. The function of the Bursa is to prevent the friction between bone and other structures like tendon, ligament etc Prepatellar Bursa is present on the anterior aspect of the knee where it sits beneath the skin. The bursa produces fluid that prevents the friction. When the bursa gets irritated due to repetitive strain, or direct blow on the anterior knee, it starts producing more fluid in order to protect the knee. That is represented as swelling on the anterior knee. These swelling can create pressure on the surrounding structures. Due to the inflammation of the Bursa, there will be pain, Swelling, Tenderness, and redness with difficulty in knee flexion. Runner's Knee:- It is also called patellofemoral pain or chondromalacia patella. It is the condition where there the articular cartilage of patella goes for deterioration and softening due to repetitive strain. The other causes can be Direct blow on the anterior knee, muscular imbalance, poor alignment, arthritis. This is mostly seen in young athletic individuals. Pain is present on the Anterior knee, Tenderness, crepitus is seen, stiffness after long hours of Immobility, and difficulty in stepping down are other clinical manifestations. Jumpers knee:- Patellar tendinopathy is called jumpers knee. It's the inflammation of the patellar tendon that creates the anterior knee pain localised to the inferior pole of the patella. Patellar tendinopathy occurs when there is chronic repetitive stress to the knee extensor mechanism. Individuals with sports activities like volleyball, and basketball are most commonly affected. Clinical features mostly include pain, stiffness after long hours of Immobility, swelling, tenderness and difficulty in staircase climbing. Causes other than repetitive stress includes the increased Q angle that puts excessive strain on the patellar tendon leading to inflammation. All of the conditions are treatable and can be managed effectively with proper Rehabilitation. #thepoetphysio #physiolife
