It’s can be understood better with the Craig’s Test. And it’s something that we can’t change (structural) so for him, we encourage him to turn his feet in. Gait analysis can be difficult, but it’s 100x easier with high resolution slow motion video! Most of what I am showing you is nearly impossible to witness unless you have the proper angle and speed. Thank goodness for technology. I would say I have pretty “normal” gait mechanics with nothing standing out as abnormal. Additionally I don’t have pain anywhere from my feet to my low back. (My neck and thoracic spine are a different story but I think that’s due to how much time I spend making content 😂😂) Take care, God Bless!

🚶🏻‍♀️ Our feet aren’t right next to each other at any time in the gait cycle, so why do we train that way? 🪜 If you’re practicing pre-gait (or gait), to get an optimal step length bilaterally your patient needs to be in a big V position (terminal stance) position. ✌🏼The big V position allows for recoil of the gastrocs (also an important reason to make sure you have dorsiflexion with the knee extended) to propel your patient’s leg farther for a longer step. 🎸 This position also allows for the toe rocker to naturally happen in pre-swing 🛟 Save this one for later .. or tag a friend who needs to know!

Hips don't lie... especially when it comes to the gait cycle. 😎🚶🏻‍♀️  Trendelenburg gait is the dropping of one hip when you are walking. Keep an eye on this patient's hips, watch the distance between the outside of her right hip and the arm of the treadmill. Then do the same on the left. Do you see a difference? 🕵️ ➡️ When she is standing on her right leg, her right hip is not doing a good job of keeping her hip in alignment which is why her left hip drops down and her right hip moves out to the side.  So now that we see it in the gait cycle, let's assess it. This foam roller hip hike is a great way to see if we CAN get that hip to engage. Often, this is a pretty big struggle on the side that's having a hard time.  Even better... this also makes for a great exercise!! A clinical example to piggy-back on @theneuroguy_dpt ‘s reel 🤗 Here are some things we did during our last session to progress gait training: 1️⃣ adding an ankle weight to the affected side: this is to utilize the principle of error augmentation to improve swing on the affected side. Increasing error improves motor learning 2️⃣ dragging a weight (10 lbs kettlebell) with non-affected side: this is to force weight-bearing on the affected side and increase stance time on that side. Notice towards the end the positive step length as she begins to take longer steps with her right lower extremity, spending more time on her left (affected side). Improve gait speed when using a walker! Most common patients I utilize this technique with ✅ Lower limb amputees ✅ Stroke ✅ Multiple Sclerosis ✅ Also works great after total knee or hip replacement for loading the limb . . 🧠 Walkers are great, safe tools to utilize, often at the beginning stages of gait training however they often cause a person to over rely on their arms for support, especially when loading weight onto the affected leg. ▫️With this tip, I am gradually and lightly pulling the walker forward to enforce a step through gait pattern and not allowing the patient to stop during right stance phase. This creates a more natural “step through” gait which improves both gait speed and loading through the affected limb! *If the patient is safe to do so, I try to progress to a rollator (or nothing) as soon as possible as this is a much more feasible device to use in the community. ❗️ Please ensure you have a tech behind the patient for safely initially, and only perform if you feel confident in the patients ability to maintain their balance ❗️ . . . #neuropt #neurorehab #neuronerds #gait #gaittraining #stroke #strokerehab #strokerecovery #limbloss
