I thought OCD meant my life was over, that I would never experience the before, that OCD was the after. I remember feeling like my brain would never function “normally” again, that I would never not have 24/7 rapid fire intrusive thoughts that terrorized me. . I was wrong. . There is an after the OCD, too ❤️ I don’t currently meet diagnostic criteria for OCD. I have soooooo much more space in my brain. I don’t experience repetitive intrusive thoughts (I do get them occasionally). I am 97% better. I am writing a memoir about my lived experience with OCD to share with the world. . One of the hardest parts of OCD is feeling like you’ve lost your identity and life forever. You haven’t. It gets better. Harm OCD is not uncommon, and can be devastating to live with, but it is rarely talked about. I have never been to any teaching on it as a doctor & although I have recognised it in several of my patients over the years, it is only recently, through a conversation with someone on this channel that I learned it was called Harm OCD. This lack of awareness, among the general public and healthcare professionals, makes it even lonelier and harder to live with when you are affected 😞 The more we talk about it, the more we will be able to help. #harmocd is an obsessional worry, characterised by intrusive, unwanted thoughts that you could harm someone. This might be worrying that you could commit an act of violence, but I have also seen the same happen with someone worrying they might harm themselves and could end up taking their own life, or worrying that they could be a paedophile. What marks Harm OCD apart from someone who actually has violent tendencies or is actively suicidal or who is at risk of paedophilia is that the thoughts appal and repel you - the actions you are worrying about are entirely contrary to your nature, which is why they cause such anxiety and stress when they pop into your head. There is nothing odd at all that weird thoughts pop into your head (we all have this sometimes, we just quickly dismiss them). But for you, the thoughts have stuck. Two problematic ways of trying to deal with these thoughts then ensure that they keep coming back. The first is reassurance seeking, and the second is avoidance. Both reinforce the problem. The more you seek to reassure yourself that you are not violent the more you are thinking about violence, and the more you avoid (eg hiding the kitchen knives) the more you lose normal, healthy associations that balance intrusive thoughts. Treatment can make a big difference here - primarily CBT and a treatment called ERP (exposure and response prevention), and there may be a role for medication (an SSRI). Has this happened to you? ✨ The cycle of OCD keeps running on the belief that if you fail to act, something bad will happen — and if it does, it will be your fault. To break the cycle, that belief has to be addressed. 
✨ Every new obsessive thought and compulsion strengthens that belief, it’s like the brain running the same track over and over. The step-by-step way through it often goes like this: 1️⃣ Recognising the belief, for example: “I am responsible for preventing harm.” 2️⃣ Gently questioning that belief: is it actually true? Looking for evidence for and against the thoughts, and accepting that thoughts don’t equal real-life events. 3️⃣ Practising sitting with the uncertainty for longer and longer without doing the compulsion the mind is pushing toward. 4️⃣ Continuing to use an approach like this gradually weakens the track, giving obsessions and compulsions less to cling to. Confessing every detail so that a person (including therapist) “really knows you” can be a compulsion for people with OCD. They may feel like if they don’t share every single thing, they are lying and the therapist will not be able to assess whether or not it’s OCD aka “if you don’t hear every detail, I can’t trust that it’s actually OCD and not you just thinking it’s OCD” . OCD specialists are aware of this and often (kindly) stop clients from going down this road 🤎 The confession is more engagement with obsessions that ultimately fuel’s OCD’s presence. . Living with undiagnosed OCD was the scariest thing to ever happen to me (by far!!!) 🥵 . If I had known that terrifying, repetitive intrusive thoughts and images were a symptom of OCD, I wouldn’t have spent 15 months suffering in silence worried that I had turned into a monster in a split second. . This is a big part of why I do what I do: to help others feel less alone and afraid, and to raise awareness about *actual* OCD ❤️ . PS experiencing intrusive thoughts doesn’t automatically mean you have OCD. People with OCD experience them repetitively and perform excessive + time-consuming mental and/or physical compulsions in response to them. .
