A 12-lead ECG records changes in electrical activity of the heart during each part of the cardiac cycle that is observed from 12 different views (leads). The conduction of the heart’s electrical impulses is detected through the skin by 10 electrodes, six placed on the chest and four on the limbs. The trace is displayed on a cardiac monitor or recorded by the ECG machine. Electrocardiogram waveform The heart muscle contracts and relaxes in response to the electrical impulse that arises in the sinoatrial node and spreads through the conduction system across the atria and ventricles. This electrical activity is recorded as the ECG waveform on graph paper. The paper is divided into small squares of 1 mm each. Standard ECG paper speed is 25 mm/s per one square (1 mm) which equals 0.04 s horizontally or 0.1 mV vertically. Each waveform deflection is related to the electrical events of depolarisation and repolarisation occurring in the cardiac tissue. A normal ECG waveform (Figures 30.2 and 30.3) consists of: P wave: should be symmetrically rounded and, except for the lead aVR, it should be positive (upright). PR interval: starts from the beginning of the P wave and ends at the beginning of the QRS complex. QRS complex: it is made up of three waves. If the first deflection of the QRS complex is downward, it is called Q wave and it represents septal depolarisation. The normal Q wave should not be wider than 1 mm and deeper than a quarter of the height of the R wave. Small Q waves may be seen in leads I, II, aVL, V5 and V6. The first upward deflection is called the R wave and it is generated by the depolarisation of the bulk of the ventricles. 🫀 گره‌های هدایتی قلب؛ سامانهٔ هماهنگ‌سازی الکتریکی میوکارد سیستم هدایتی قلب شبکه‌ای از سلول‌های تخصص‌یافتهٔ میوکاردی است که مسئول تولید و انتقال پتانسیل‌های الکتریکی‌اند تا انقباض دهلیز و بطن با نظمی دقیق انجام گیرد. این سامانه از چهار جزء اصلی شکل گرفته است که مسیر ریتم سینوسی طبیعی را می‌سازند: --- ۱. گره سینوسی (SA Node) در دیوارهٔ فوقانی دهلیز راست، منشاء اصلی ریتم قلبی است. سلول‌های آن فاقد پتانسیل استراحت پایدارند و به‌طور خودکار دپولاریزاسیون تدریجی ایجاد می‌کنند. جریان یونی Na⁺ و Ca²⁺ سبب پدید آمدن موج **P** در ECG می‌شود. --- ۲. گره دهلیزی–بطنی (AV Node) در سپتوم بین‌دهلیزی قرار دارد و تنها مسیر فیزیولوژیک انتقال پیام از دهلیزها به بطن‌هاست. هدایت در این گره با **تأخیر کوتاه (حدود ۰٫۱ ثانیه)** انجام می‌شود تا دهلیز پیش از انقباض بطنی تخلیه گردد. این فاز با **PR interval** متناظر است. --- ۲. دستهٔ هیس و شاخه‌های چپ و راست پس از گره AV، تحریک وارد دستهٔ هیس شده و در سپتوم بطنی به دو شاخهٔ اصلی تقسیم می‌شود؛ این مسیر با هدایت سریع جریان، زمینهٔ دپولاریزاسیون هم‌زمان بطن‌ها را فراهم می‌کند. --- ۴. شبکهٔ پورکینژ (Purkinje Fibers) الیاف انتهایی با قطر بزرگ و Gap Junction فراوان که انتقال پتانسیل را با بیشترین سرعت هدایت می‌کنند. این مرحله منجر به موج **QRS** در نوار قلبی و انقباض یکپارچه‌ی بطن‌ها می‌شود. --- در مجموع، توالی SA→AV→His–Purkinje مدار دقیق الکتریکی قلب را می‌سازد. هر اختلال در این مسیر (مانند بلوک AV یا آسیب ایسکمیک SA) به آریتمی منجر می‌شود.
