Nephrotic Syndrome is a clinical condition characterized by: Massive proteinuria (>40 mg/mยฒ/hr or urine protein/creatinine ratio >2) Hypoalbuminemia (<3 g/dL) Edema Hyperlipidemia Etiology:- Primary (Idiopathic): Minimal Change Disease (MCD) โ€“ most common (โ‰ˆ90% cases in children) Focal Segmental Glomerulosclerosis (FSGS) Membranoproliferative GN Secondary: SLE Infections (Hepatitis B, malaria) Drugs (NSAIDs) Allergies, malignancies Clinical Features:- Edema: Periorbital โ†’ generalized (ascites, pleural effusion) Oliguria Frothy urine No hematuria or hypertension (in MCD) Infections due to loss of IgG and complement Investigations:- Urine: Proteinuria (+++/++++), few cells Serum: โ†“ Albumin, โ†‘ Cholesterol Renal function: Usually normal Urine microscopy: Bland Renal biopsy: Only if atypical features (age <1 yr, >10 yr, hematuria, HTN, renal failure) Treatment:- General measures: Salt restriction Diuretics (for edema) Treat infections Specific: Corticosteroids: Prednisolone 2 mg/kg/day ร— 6 wks, then alternate days ร— 6 wks Steroid-resistant โ†’ Calcineurin inhibitors (cyclosporine/tacrolimus) Prevention of complications: Pneumococcal & influenza vaccines Complications:- Infections (peritonitis, cellulitis) Hypovolemia Thrombosis Relapses Steroid toxicity Prognosis:- Minimal Change Disease: Excellent response (>90% remission) FSGS: Poor response, may progress to CKD
