Ascending (Sensory) Tracts These carry sensory information from the body to the brain. 1. Dorsal Column–Medial Lemniscal Pathway (DCML) Fasciculus gracilis� Fasciculus cuneatus �Carries: fine touch, vibration, proprioception� 2. Spinothalamic Tracts Anterior spinothalamic tract� Lateral spinothalamic tract �Carries: crude touch (anterior), pain and temperature (lateral)� 3. Spinocerebellar Tracts Dorsal spinocerebellar tract� Ventral spinocerebellar tract� Cuneocerebellar tract� Rostral spinocerebellar tract �Carries: unconscious proprioception to the cerebellum� 4. Anatomical ascending somatosensory pathways of the spinothalamic tract(STT),the dorsal column-medial lemnis- cus(DCML)& the trigeminal thalamic tract(TTT)which are all formed by long-projecting myelinated fibers originated from the spinal dorsal horn(red dot & line),nuclei gracilis & cuneatus(blue dots & line),sensory nuclei of the trigeminal nerves(green dots & line)& send projections to the ventroposteromedial & ventroposterolateral nuclei of the thalamus(VPM/VPL),which relay the somatosensory information to the somatosensory cortices.The trigeminal & spinal ganglia contain the pseudounipolar primary sensory neurons which send their peripheral branches innervating the head,the face,& the whole body,& their central branches to the trigeminal nuclei in the brain stem & the spinal dorsal horns & the dorsal column nuclei in the medulla oblongata.Pain, a complex,unpleasant sensory& emotional experience,serves as a crucial biological warning system,alerting the body to actual or potential harm.In the International Classification of Diseases 11th Revision (ICD-11),chronic neuropathic pain is specifically categorized under ‘‘MG30 Chronic Pain’’ & refers to neuropathic pain that persists for more than 3months. Unlike nociceptive pain,which arises from tissue injury or inflammation,chronic neuropathic pain is characterized by abnormal processing of sensory input within the nervous system.In mammals,the nociceptive system is the part of the nervous system responsible for detecting& responding to potentially harmful stimuli.These nociceptors convert the harmful stimulus into electrical signals through a process known as transduction.The signal is then transmitted via primary afferent fibers(Aδ & C fibers)to the spinal dorsal horn(SDH),where it is processed& modulated.
