DOCrown Preparation
1. Occlusal reduction – Crown material ke liye space. Metal crown ~1.5 mm, PFM ~2 mm, All ceramic ~2 mm.
Over-reduction se tooth weak ho jata hai, under-reduction se crown bulky aur occlusal interference hota hai.
2. Functional cusp bevel – Mandibular buccal cusps aur maxillary palatal cusps par diya jata hai. Isse strength aur proper clearance milta hai. Agar bevel na ho toh crown thin hoga aur fracture ho sakta hai.
3. Axial reduction – Uniform hona chahiye. Metal ceramic 1.2–1.5 mm, all ceramic 1.5–2 mm. Tapered round-end diamond bur use hota hai. Over-taper se retention kam ho jata hai.
4. Finish line – All ceramic ke liye shoulder, metal ke liye chamfer, aur PFM ke liye shoulder with bevel. Finish line smooth aur continuous honi chahiye, ledges avoid karna hai.
5. Taper / convergence angle – Ideally 6–12°. Excess taper se retention aur resistance dono reduce ho jate hain.
6. Retention & resistance form – Parallel walls se retention zyada hota hai. Height:width ratio ideal 3:1. Agar tooth short hai toh grooves ya boxes diye ja sakte hain.
7. Marginal integrity – Smooth aur continuous margin hona chahiye. Unsupported enamel avoid karna hai, warna marginal leakage ho sakta hai.
8. Surface finish – Sab walls smooth honi chahiye, sharp edges remove karne hote hain, taki stress concentration aur fracture na ho.
Clinical tips: Occlusal clearance wax/putty se check karo. Under-reduction → bulky crown & occlusal interference. Over-reduction → weak tooth, pulp exposure. Moisture control essential hai.

ECBicuspid Aortic Valve – R-N Fusion
R-N fusion is a type of bicuspid aortic valve where the Right Coronary Cusp (RCC) is fused with the Non-Coronary Cusp (NCC), forming a functional bicuspid valve.
🔍 Echo Findings (Parasternal Short Axis View):
Two cusps instead of three
Raphe seen between RCC & NCC
Asymmetric cusp opening (“fish-mouth” appearance)
Eccentric valve orifice
⚠️ Clinical Significance:
Can lead to Aortic Stenosis or Aortic Regurgitation
Associated with ascending aortic dilatation
Important for surgical and follow-up planning
💡 Tip for Identification: 👉 Carefully trace cusp margins and look for the raphe line to confirm fusion pattern
