Insertion: Dorsal aspects of bases of both the middle and distal phalanges of the index, middle, ring, and little fingers. Action: Extends index, middle, ring, and little fingers. Innervation: Posterior antebrachial interosseous nerve (C7-C8). The common extensor tendon is the combined tendon for four of the six superficial muscles in the posterior compartment of the forearm, which are the: - extensor carpi radialis brevis muscle; - extensor digitorum muscle; - extensor digiti minimi muscle; - extensor carpi ulnaris muscle. The extensor digitorum muscle is located: - Posterior (superficial) to the supinator, abductor pollicis longus, extensor pollicis longus, and extensor indicis muscles; - Medial to the extensor carpi radialis brevis muscle. - Lateral to the extensor digiti minimi and extensor carpi ulnaris muscles. Each of these muscles are part of the superficial posterior compartment of the forearm and extend the wrist and fingers, abduct and adduct the hand, and aid in forearm supination. Common extensor tendon tendinopathy, commonly called tennis elbow, is seen in racket athletes as well as people who use twisting and wrist extension repetitively, like using a screwdriver. Here’s why this matters a lot: When we bend our knee, the femur and tibia (leg and shin) need to internally rotate. This is a necessary mechanism that allows for our knee to properly manage and distribute pressures/forces evenly throughout our legs as we load them. The problem is, this knee presentation represents a knee that is stuck in external rotation. This means that the mechanics of the knee bending and the shin moving forward over the toes are going to be compromised. So it’s very common to see people that have knee pain with knee bending, such as going down stairs, with this type of visual knee presentation. Also, the knee bending, and the foot arch subsequently lowering into the ground in pronation, is necessary for us to effectively shift our center of gravity from side to side. So the knee that can’t effectively bend it is usually the side we can’t push out of effectively. Restoring genuine motions of the ability to knee to bend will not only allow us to better manage forces in a more efficient way, but also allow us to restore better management of shifting from side to side within our center of gravity with every step we take.

Like other tendon issues, resistance training appears to be helpful in many cases for both reducing painful symptoms and improving the work capacity of the tendon. Here are several strategies that you can try if you are currently experiencing pain in this region. 1️⃣ Wrist Extensor Muscle Stretch: This position stretches the wrist extensor muscles and can help relieve pain and soreness. Perform 2-4 repetitions and hold for 15-30 seconds. 2️⃣Radial Nerve Mobilization: The radial nerve runs through the extensor compartment of the forearm and may be associated with tennis elbow type pain. The specific movement shown here will put the radial nerve on tension and can be a useful technique for decreasing any nerve sensitivity in the region. 3️⃣Wrist Extensor Isometric: Start here if your pain is severe. Perform 4-5 repetitions each day and hold the position for 30 to 45 seconds to help ease pain and begin strengthening. 4️⃣Wrist Extensor Curl: Skip number 3 if you can perform this movement with no more than mild (3/10) pain. Move through the full range of motion, which will work the wrist extensors both concentrically and eccentrically. Shoot for 3 sets of 6-12 repetitions. 5️⃣Supinator Curl: This exercise will target the supinator muscle, which also attaches at the lateral elbow.
