MA📌الکتروشوک درمانی (ECT) یک روش درمانی است که با استفاده از جریان الکتریکی کوتاه مدت به مغز اعمال میشود. این روش معمولاً برای درمان اختلالات روانی جدی مانند افسردگی شدید، اسکیزوفرنی، و اختلال دوقطبی استفاده میشود، به ویژه زمانی که سایر روشهای درمانی مؤثر نبودهاند.

SC1. ECT (Electroconvulsive Therapy) is a psychiatric treatment using a controlled electric current to induce a therapeutic seizure.
2. It is mainly used for severe depression with suicidal tendency and treatment-resistant depression.
3. Other indications include catatonia, mania, schizophrenia with catatonic features.
4. Absolute contraindication: Raised intracranial pressure (ICP).
5. Relative contraindications: Recent MI, uncontrolled hypertension, aneurysm, severe cardiac disease.
6. Modified ECT is standard—given with anesthesia and muscle relaxant (succinylcholine).
7. Procedure: NPO 6–8 hrs, consent, premedication, electrode placement, current 70–120V.
8. Bilateral ECT works faster, but causes more memory loss; unilateral has fewer cognitive effects.
9. Side effects: Headache, confusion, transient memory loss, retrograde amnesia.
10. Fastest improvement seen in acute suicidal depression & catatonia.

KRDEPRESSION AND BRAIN VOLUME CHANGES AFTER ECT vs TMS
Treatment resistant depression (TRD) has been treated with both electroconvulsive therapy (ECT) as well as repetitive transcranial magnetic stimulation (rTMS).
Cano et al (1) studied 32 patients with TRD, and used structural magnetic resonance imaging (MRI) to evaluate brain volume changes after either: 1) right unilateral ECT, or 2) rTMS applied to the left dorsolateral prefrontal cortex ( DLPFC ). They found that although the clinical outcomes were similar, ECT was associated with a structural change while rTMS was not. More specifically, the RUL ECT treated patients showed increased volumes in the right striatum, pallium, medial temporal lobe, anterior insular cortex, anterior midbrain, and subgenual anterior cingulate cortex. The authors suggest implications of clinical outcomes as follows:

rTMS may be associated with neurophysiological plasticity

الکتروشوک درمانی (ECT) در دهه ۱۹۴۰ به عنوان یکی از روشهای اصلی درمان برای بیماران روانپزشکی بهویژه کسانی که از اختلالات شدید روانی مانند افسردگی شدید، شیدایی، و اسکیزوفرنی رنج میبردند، مورد استفاده قرار گرفت. این روش بر پایهی ایدهای شکل گرفت که شوک الکتریکی میتواند باعث تغییرات مثبت در عملکرد مغز بیماران روانی شود.
روش ECT در این دوره به این صورت بود که پزشکان از طریق اتصال الکترودهایی به سر بیمار، جریان الکتریکی را از مغز عبور میدادند. این شوک الکتریکی، به طور موقت فعالیت نورونها را مختل کرده و به ایجاد یک حمله تشنجی منجر میشد. فرضیهای که پشت این درمان بود این بود که این حملات تشنجی میتواند تعادل شیمیایی مغز را بازگرداند و علائم روانی را کاهش دهد.

به طور کلی، در دهه ۱۹۴۰، الکتروشوک درمانی به عنوان یک گزینه درمانی مؤثر شناخته میشد، هرچند که مشکلات و چالشهای زیادی به همراه داشت.
