Thomas Test: A quick test used to help determine the cause of limited hip extension ROM (hip flexor tightness). As the innominate (ASIS) moves, observe the thigh angle relative to the table (parallel): ✅ Negative Test Thigh rests parallel on the table with the knee flexed. ❌ Positive Test– Rectus Femoris Thigh reaches parallel only after the knee is straightened. ❌ Positive Test– Iliopsoas (Iliacus + psoas) Thigh position does not change, regardless of whether the knee is bent or straightened. ❌ Positive Test– Both (Iliopsoas & Rectus Femoris) Thigh lowers further after knee extension but still does not reach parallel to table. *TFL involvement: Determined by how much the thigh abducts (moves off midline) during lowering. However, this is a bit more nuanced, thus, was not shown.

A quick test I’ll do with tight hip flexors (confirmed by a Thomas test) is to go right into a few reps of a contract-relax technique to see if I can immediately buy some hip extension range of motion. Essentially, I’m testing to see how malleable the range of motion is, and if most of the tension is actually being generated from the nervous system. If this immediately changes hip extension and buys me a lot more range of motion, then I know I’m dealing mostly with a motor control issue, and I’ll likely see the biggest improvement with a little bit of control training in that range, on both sides of the joint. . #ManualTherapy #MassageTherapy #SportsTherapy #NeuromuscularTechniques #MET

Most people think running issues come from weak legs or poor mechanics but sometimes the problem starts at the hip. In this video, I broke down my own movement analysis to understand why my sprint mechanics have been off. When I run, my hip drops when my foot hits the ground especially when my right foot lands. That hip drop changes how force transfers through my body and makes it harder to stay low during acceleration. Instead of projecting forward, I pop upright too early. After testing my mobility, here’s what stood out: • Hip flexors weren’t the issue my Thomas test showed improvement and less tightness. • Right hip = very limited internal rotation. • Left hip = limited external rotation. • This imbalance changes how my pelvis rotates and how I load each side during sprinting. Because my right hip lacks internal rotation, my body has a harder time accepting force efficiently when that leg hits the ground. Instead of rotating smoothly through the hip, compensation happens through the lower back, glute, and hamstring attachment. That’s why the tightness and pain keep showing up mostly on my right side. When the hip can’t move well, the body steals motion from somewhere else. For me, that “somewhere else” has been: • Lower back tightness • Right glute tension • Hamstring insertion discomfort • Hip drop during running • Losing acceleration posture and popping upright too early Mobility isn’t just flexibility it’s access to positions your body needs for speed, force production, and efficient movement. Sometimes fixing your sprint mechanics starts long before you even take your first step. Working on how to clean this up and improve my hip mobility.
