Gaze stabilization exercises are a cornerstone of vestibular rehabilitation — especially after concussion. The goal is to keep your vision clear and focused while your head is moving, which requires your vestibulo-ocular reflex (VOR) to be firing on all cylinders 🎯 But once a patient has nailed this on flat, stable ground, that doesn’t mean the work is done. In the real world, we don’t always walk on perfectly even surfaces. We step off curbs, navigate uneven terrain, and move through environments that constantly challenge our balance 🌍 That’s where progression comes in 📈 By adding an unstable surface like a Bosu ball, we’re asking the brain to do more with the same exercise. Now the vestibular system isn’t just stabilizing your gaze — it’s also integrating somatosensory information to keep you upright. The brain has to process more, prioritize faster, and coordinate multiple systems at once 🤯 That’s exactly the kind of multisensory demand that people with post-concussion dizziness struggle with in everyday life. You don’t need an entirely new exercise every time a patient progresses. Just think about how you’re manipulating the environment around the task ⬇️ 🔄 Change the surface 🧩 Add a dual task 👁️ Alter visual complexity ⚡ Modify head movement speed If you’re a clinician working with post-concussion vestibular dysfunction, think about how you’re layering your progressions. Has anyone ever assessed your vestibular system… or just your knee? If you can’t stay stacked on one leg, lose balance easily, or feel unstable when speed increases… that’s not always a strength or mobility issue. Poor stacking is often a vestibular issue. The semicircular canals (SSC) are part of your vestibular system. They detect head rotation, speed, acceleration, and deceleration, and help stabilize your vision. In this progression, we challenge the SSC with rapid head movements during single-leg balance. Level 1: Eyes open → Vision helps while the vestibular system starts organizing movement. Level 2: Eyes closed → Vision removed. The brain must rely on vestibular + proprioceptive input to stay stacked. Why this matters 👇 When the semicircular canals aren’t doing their job: → delayed postural responses → loss of stacking → force leaks into the knee Train the system that organizes movement, not just the muscles that react to it. I like to create chaos in Concussion rehab but also with athletes recovering from MSK injuries as well 🧠⚡️ When we add dual-task or cognitive load (decision-making, reacting, divided attention) to balance, cutting or landing… ankle control often drops. In chronic ankle instability (CAI), research shows: 👉 Larger balance errors under dual task 👉 Slower peroneal activation 👉 Greater sway during gait 👉 Bigger “dual-task cost” compared to healthy controls (Burcal et al., 2019; Abdallat et al., 2020) Sport is never single-task. ⚽ Scan + cut 🏀 Catch + land 🏉 React + absorb contact If rehab is quiet and predictable, we’re underdosing reality. The encouraging part? It’s trainable. A 6-week balance + cognitive program in CAI improved: ✔️ Proprioception ✔️ Peroneus longus onset timing ✔️ Dynamic balance ✔️ Cortical efficiency More than balance training alone (Chai et al., 2024). So modern ankle rehab should progress: 1️⃣ Single-task control 2️⃣ Add light cognitive load 3️⃣ Add reactive visual cues 4️⃣ Unanticipated cutting & landing 5️⃣ Fatigue + divided attention testing Train the ankle. Corrective exercise is neither corrective… nor is it exercise. It’s often just adult tummy time with a new sales pitch. And it trained the industry to do one thing really well: Find “asymmetry” and call it pathology. Most asymmetries are NORMAL. Dominance. Sport bias. Anatomy. Life. Michael Phelps would get roasted on a posture screen… while having proportions that were literally an advantage in the pool. Usain Bolt talked about his scoliosis… and still became the fastest human alive. Asymmetry only matters when it changes capacity and raises risk in specific populations: Ankle DF asymmetry ≥6.5° linked to higher injury odds in Army Rangers. (Teyhen et al., CORR, 2015) Pitchers: >5° total motion deficit linked to higher shoulder/elbow injury risk. (Wilk et al., AJSM, 2011; 2014) SEBT anterior reach >4 cm difference predicted higher LE injury risk in HS basketball. (Plisky et al., JOSPT, 2006) Scapular dyskinesis associated with future shoulder pain risk (SR/MA). (Hickey et al., BJSM, 2018) Notice the populations here- high level performers with high level requirements. Not someone just getting off the couch or wanting to lose 5lbs. If your “correctives” never load the system… you’re not changing the system. Gravity doesn’t care about your floor routine. The best corrective is usually training the ACTUAL movement and customizing the set up. That’s it Don’t sell symmetry and call it training. Isometric tests may look simple, but without close monitoring, the data can be very misleading. When using isometric tests a good setup is essential, but it isn’t always sufficient to guarantee data quality. You also need to inspect the force–time curve. With isometric tests, fixation can easily be “almost” secure. But subtle movement that aren’t always obvious during the test can introduce jerk and artefact into the force trace. See the example in the video. These artefacts can inflate peak force and have an even bigger effect on rapid force measures (e.g. 100–150 ms). This can make your data nothing more than a random number. Definitely not something you want if you’re using tests to inform rehab or return to play post injury. That’s why monitoring force–time data is essential. If the force trace isn’t clean. Adjust the setup. Improve fixation. Repeat the test. Clean traces give us more meaningful data we can be confident with. #sportsrehab #sportsmedicine #returntoplay
