In the clinic we see a ton of injuries that start long before the athlete hits the field. Most often it is not one thing that breaks down … it is a pattern. The good news is that many of those patterns are preventable. As physical therapists we don’t just treat the painful spot, we look at the whole lower extremity system, how muscles work together, and how your training matches your sport. You should think like this when stretching as well. Athletes often focus on tight hamstrings or quads, but if you are only stretching one muscle you are leaving the rest of the kinetic chain vulnerable. Remember to stretch all aspects of the lower extremities to avoid injury. That is why when we evaluate an athlete we look at: • joint mobility and range of motion • muscle flexibility across the entire leg • strength and neuromuscular control • movement patterns specific to their sport Then we build a customized plan with sport-specific mobility and strengthening. For example (and this is not an exhaustive list nor does it apply across all individuals) • a runner may need dynamic hip and calf mobility for efficient stride mechanics • a soccer player may need coordinated hip, knee and ankle stability for cutting and pivoting • a basketball player may need ankle mobility and quad-hip balance for jumping mechanics Stretching and mobility should serve your sport-specific demands not just one muscle. Consistent stretching with strength and movement integration reduces injury risk, improves motion, and sets you up for better performance long term. Hip Flexors - Strengthen & Lengthen - Level 1: Overcoming Isometrics x 3 positions Most people have heard of “tight hip flexors” and maybe you have been told that you have tight hip flexors yourself! This is an exercise that I recently learned from @coachrobmauceri (I recommend you go check out his podcast w/ @chrisabeardsley - high performance physiology) and I have started to prescribe for low back, hip, and knee issues. This is a super accessible version that you can do at home, in the changing room or at the gym. Early stage pain & injury rehab - 2-3 times a day, every day. Footballers, runners and lifters - put this into your warm ups before sessions! Once the pain settles you can then progress through the various levels that I’ll share with you, and load them up properly twice a week. Do a 5 sec hold in each position, driving your knee as hard as you can, and repeat through each position 3-5 times. Your hamstrings may not be “tight.” They may just be weak. But tightness doesn’t always mean a muscle needs stretching. Sometimes, tightness is actually your body asking for strength and control. I have a patient who had hamstring surgery about five months ago. Early in his recovery, the goal was helping the tissue heal and remodel. That included a lot of myofascial work and scraping to help manage scar tissue and improve tissue remodeling During that phase, it helped a lot. But recovery changes as the body heals. Now he’s cleared to return to!a physically demanding job and wants to get back to hooping on the weekends. After preforming those activities, he feels a pulling or tight sensation in the hamstring, and his first thought is: “Maybe I just need more scraping.” But at this stage, the tightness isn’t coming from scar tissue. It’s coming from a hamstring that’s being asked to produce force and absorb load again. When muscles return to explosive activity like sprinting, jumping, heavy lifting, they can feel tight because they’re not yet strong enough for the demand being placed on them. More passive treatment won’t fix that. What the tissue needs now is progressive strengthening. Building the capacity of the hamstring to handle load, speed, and explosive movement again. Because the goal after surgery isn’t just to make the muscle feel better. The goal is to restore its ability to perform. So if a muscle feels tight, the answer isn’t always stretching. Sometimes the real solution is building the strength and resilience that muscle needs to do its job again.

Using a band is useful in this scenario as I can adjust the distance he is from the anchor to create max effort as the accommodating resistance builds to match his strength eventually finding the point of a max effort isometric. Prone is useful as the rectus femoris is a bi-articulate muscle. If you are seated the rectus femoris fem is in a shortened position. By being prone it extends the hip which lengthens rec fem. Rec fem puts direct pressure on patellar tendon therefore it is very useful to target directly. Do you use isometrics in your training/rehab? #sportsphysicaltherapy #kneepain

It’s always interesting to me as a sports PT having an injury I’ve never had before and navigating it, learning all the things I can and can’t do. Case in point: hip extension - with that leg straight, there’s lots of tension on the proximal HS tendon to try and lift the leg up for hip extension, so, it’s painful, and also I just can’t. I tried to assist with my left leg to see if I could passively lift my leg and hold it (= get past the initial stress and then glute max can take over with the assist of the HS), but I didn’t realize how flexed my right knee was, so I was sort of unknowingly cheating.
